25 | Licensing Program Analyst (LPA) Lilia Hernandez conducted an unannounced case management inspection on 09/20/2024 due to self reported incidences that occurred at the facility. LPA arrived at the facility at 1:30PM and met with Carmen Vasquez, Principal. There were 49 children and 18 staff present upon arrival.
The incident that occurred on 08/12/2024 was reported to the Department on 08/14/2024, via telephone. The facility did not reported the Unusual Incident to the Department within the required 24 hours of occurrence.
Information reported to the Department indicated that interactions between Staff #1 and Staff #2 may or may not have violated the personal rights of children in care.
LPA obtained a copy of the facility roster and other pertinent information regarding this incident. LPA was not able to conduct interviews with Staff #1 and Staff #2. Staff #1 and Staff #2 were not present during the inspection. Additional information is needed to determined if the personal rights of children in care were violated.
The incident that occurred on 08/14/2024 was reported to the Department on 08/14/2024, via telephone. The facility reported the Unusual Incident to the Department within the required 24 hours of occurrence.
Information reported to the Department indicated that Staff #3 may or may not have violated the personal rights of Child #1. LPA interviewed Principal and Staff #3 during today's inspection. Based on the information LPA obtained from the interviews, it was determined that the personal rights of Child #1 were not violated.
