25 | Licensing Program Analyst (LPA) Alicia Bailey conducted an unannounced random annual (or Required) inspection. LPA met with Licensee Charlotte Polk and assistant at 9:08 am who guided the LPA on a tour of the facility. At 9:15 am during the inspection LPA Bailey discussed, assessed and noted individuals residing in the home. The facility Entrance Checklist for Family Child Care Homes was given to Licensee Polk. This is a one story home consists of 3 bedrooms and 3 bathrooms, children room, attach garage, front yard and backyard. Areas used living room, one (1) restroom, one bedroom, kitchen, children room and front yard. Per Licensee Polk and facility sketch, areas off limits to children include: 2 (two) bedrooms, two bathrooms and garage .The home was inspected for safety, comfort, cleanliness, telephone service, heating and ventilation, inaccessibility to poisons, detergents, cleaning compounds, medicines, and hazardous items that can pose a danger to children. The Licensee Polk stated no other license is held at the the facility. There are 11 children present during time of inspection. Licensee Polk states that there is currently 15 children enrolled in the facility. Facility hours of operation Monday thru Friday 6:00am to 4:59am 24 hours. The facility ages 0 years old to 12 years old.
At 9:30 am LPA Bailey ask licensee Polk do you have a firearm, licensee Polk state no. LPA Bailey ask licensee Polk do you own a pet licensee Polk state yes, we keep the dog lock in room away from the day care children
At 9:50 am LPA Bailey observed a large television mounted to the wall in the children room. LPA Bailey observed , age appropriate toys and napping equipment. At 10:00am LPA Bailey tour the children bathroom. LPA Bailey did not observe any hazardous items in the bathroom.
