On 12/7/2022 Licensing Program Analyst (LPA) Judy Laureano conducted an unannounced Annual Required Inspection at Dvorskaya Alternative School DBA: West Hollywood College Preparatory School located at 1317 N. Cresent Heights Blvd. Los Angeles, CA 90046 and met with Director Jina Mukhuradze.

The school age program is housed upstairs above the preschool program. LPA toured the facility indoors and outdoors. No children were present during today’s inspections. Entrance to the facility is located on Fountain Blvd.; the side of the facility. LPA observed a sign in and out sheet that is used by the on site private school in conjuction with the afterschool program. LPA advised director to review sign in sheet and ensure afterschool participants can me clearly identified.

LPA observed the upstairs to have 8 rooms, 5 used as classrooms, 3 used as offices and a kitchen area. Days and hours of operation are Monday through Friday 2:00 p.m. to 6:00 p.m. Currently facility has 18 children enrolled and do not have anyone enrolled past 4:30 p.m.

Licensee also has an Affidavit for their private school, however the rooms that are currently being used are those of the after school age program. The private school operates 9am to 1:45 pm and the school age children arrive at 2pm to 4:30 p.m. Licensee was reminded that the Private school children can not co-mingle with the after school children.

There is no swimming pool or other bodies of water on the premises.

Medications are made inaccessible and stored in the kitchen in the second floor, top cabinet. Facility does not have any children who are prescribed medication. If medication needs to be refrigerated, medication will be stored in kitchen with all necessary documents. LPA reminded director that all medication needs to be made inaccessible to the children in care.

Teachers do the day to day cleaning in the classroom. Outside pest control vendor is contracted to ensure facility stays pest free. Facility is free of flies, insects and rodents. No poisons were observed in today’s inspection.

Disinfectants, cleaning solutions and other hazardous items are made inaccessible in the preschool kitchen.

Fire extinguishers and evacuation plans are in each classroom.



Facility provides snacks. Menu is posted on parent board and emailed to parents. The preparation area is in the preschool kitchen and once the children's snack is ready it is taken upstairs to the children. Evening dinner is also available to those children who request it. All food is protected against contamination and any contaminated food is discarded immediately. Sharp pointed objects are inaccessible to the children.

Furniture and equipment are in good condition, free of sharp, loose or pointed parts. Playground equipment is in safe condition, free of sharp, loose or pointed parts. Licensee removed three outdoor bicycles due to missing handlebar covers. They will be inaccessible to children until the equipment is repaired. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. School age children do not comingle with preschool children when using the outdoor yard.

All toilets and handwashing facilities are in safe and sanitary operating condition. Bathroom facilities are located in the first floor. Floors in the facility are clean and safe. All kitchen, food preparation and storage areas are clean, free of litter/rubbish and free of rodents/vermin.

Drinking water is available both indoors and outdoors. LPA observed a water dispenser outside the kitchen in the second floor area where children and teachers are able to access water when needed. When outdoors, teachers take a water pitcher with cups for children to use.

Areas around high climbing equipment, swings and slides have cushioning material to absorb falls. The facility is free of flies, insects and rodents. Facility has a sprinkler system, smoke detector system and carbon monoxide detector in each classroom.

Prior to working or volunteering in a licensed childcare facility, all individuals subject to a criminal record review have received a criminal record clearance or exemption. Upon notification from the Department, the licensee will comply and act immediately to terminate the employment of, remove from the facility or bar from entering the facility for any person it is deemed necessary while the Department considers granting or denying an exemption. Capacity and limitations as specified on the license are being maintained.

At least one person trained in CPR and Pediatric First Aid is present when children are at the facility or at offsite activities. LPA reviewed Mandated Reporter Training and staff has current proof of completion, Director completed Mandated Reporter Training on 9/26/2022. Director’s CPR and First Aid was completed on 1/16/2022. LPA reviewed staff files and 1 out of 2 files were observed to be incomplete. LPA reviewed staff files and a technical violation was issues. Director agrees to ensure all files are complete with all the necessary LIC forms and educational requirements.

LPA reviewed a sample of children’s files and observed 1 out of 5 files were incomplete. LPA provided director with a copy of the LIC 311A to use as a reference to ensure all files have current documents.

The name of the child care center director or fully-qualified teacher(s) designated to act in the director’s absence has been reported to the Department. The person who signs the child in/out of the facility shall use their full legal signature and record the time of day..

All children are under supervision, including visual supervision, of a teacher at all times. Facility maintains a ratio of one teacher supervising no more than 14 children in care.

Facility representative, Jina Mukhuradze was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Incidental Medical Services (IMS)currently being provided. Licensee is aware that an IMS plan is required to be submitted to the licensing office if they provide any of these services. Information regarding Americans with Disability Act (ADA) can be obtained by contacting US Department of Justice toll free ADA Information line at (800) 514-0301(voice), (800) 514-0383 (TDD) and website link https://www.ada.gov/childqanda.htm.



LPA and Director discussed the Community Care Licensing website www.ccld.ca.gov which will provide access to Provider Information Notices (PINs), Quarterly Updates, COVID-19 Information and Resources, Mandated Reporter Training, Safe Sleep in Child Care, Lead Poisoning Facts, Forms and Regulations.

A notice of site visit was given and must remain posted for 30 days.

Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
