Licensing Program Analyst (LPA) Silva Garibyan conducted an unannounced Required - 3 Year inspection on this date. LPA met with Larisa Nikogosyan, Director, and explained the purpose of the inspection and was guided on a tour of the facility. Facility is currently licensed for a capacity of 82 children. LPA observed 26 children and six teachers present at the time of the visit. Sign in/out sheet confirmed this to be accurate. All children are under supervision, including visual supervision of a teacher at all times. Capacity and limitations as specified on the license are being maintained. A walk through of the classrooms was conducted. The facility consists of five classrooms. The facility operates Monday through Friday, from 8:00 a.m. to 5:30 p.m. Facility is operating within capacity limitations. There are no bodies of water on the premises. There are no firearms or ammunition allowed or stored on the premises. Required posting were observed in classrooms and Director's office for public viewing. Facility has a working phone land line available. Disinfectants, cleaning solutions, medications, and other hazardous items are made inaccessible (stored in Director's office). A walk through of the classrooms was conducted. The classrooms were found to be clean and free from any potential hazards. Furniture was found to be in good repair and age appropriate. There is adequate heating, lighting and ventilation. Isolation area for sick students is in the Director’s office. LPA observed adequate arrangements for isolation and care of ill children. Page 1

The bathrooms were inspected. LPA observed toilets and sinks to accommodate the facility’s capacity. Facility is equipped with three toilets and three sinks located in Classroom #5 and three toilets and three sinks located in the hallway by Classrooms #1, #2, #3. Toilets flush properly, toilet and sinks are reachable by the children in care.The bathrooms have adequate toilet paper and paper towels available. LPA observed the hot water to be turned off. The bathrooms were found to be clean. There is adequate lighting/ventilation in the bathroom areas.
Furniture and equipment are in good condition, free of sharp, loose or pointed parts. Playground equipment is in safe condition, free of sharp, loose or pointed parts. All toilets and hand washing facilities are in safe and sanitary operating condition. Floors in the facility are clean and safe.

There are 2 play yards. Both areas were observed to be free of debris. There are areas for shade and rest on both yards. Drinking water is available. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. There is wrought iron and chain link fences around the playgrounds.

Lunches are prepared by the school. School also provides an AM and PM snack for the children in care. Food preparation area is not accessible to children in care. Lunch is served in classrooms. Menus are posted for parents to view. Food preparation area is adequately equipped, clean and free from hazards. Cleaning supplies are out reach of children and is stored separately and away from food. There is adequate food for meals and snacks. There is hot and cold running water in the kitchen/food preparation area. Refrigerator is clean and operating at the proper temperatures. Solid waste storage containers have tight-fitting covers and are in good repair. Page 2

The facility is equipped with a working smoke detectors and Carbon Monoxide detector. LPA observed seven fire extinguishers (3A 40BC) located throughout the facility with service date of 02/28/2024. First Aid Kit was observed to be complete. There are first aid supplies in each classroom and in the office. Fire Drill log was observed to be complete and current. Trash cans were observed with appropriate lids.

The facility has sufficient cots for napping. Each cot is occupied by only one child at time. CPR, Pediatric First Aid (completed on 06/14/2024), and Mandated Reporter Training certificates ( completed on 08/12/2022) were observed to be current. The name of the child care center director or fully qualified teacher designated to act in the director’s absence has been reported to the Department. LPA reviewed a sample of staff and children files and observed files were complete. The children's roster was available and current. LPA observed the required documents posted at the facility.

Director was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.
Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for
drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP).
LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP.
Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. A notice of site visit was given and must remain posted for 30 days.
