When interviewed, S1, S2, and S3 stated that the ceiling of the closet located in the classroom collapsed due to the rain. There was visible mold in the closet, but the children did not have access to it. The director notified the church, which then hired a mold-inspecting company. The company detected mold at the school, and the school followed the company's remediation requirements. On May 24, the school went through a post-remediation inspection, and debris or visible mold growth was not detected. S1 and S2 explained that they notified the parents of the mold following the test results from the mold inspection company on 05/09/2024. The Unusual Incident Report (LIC624) was provided during the department's first visit on 05/13/2024

Based upon interviews conducted, observations, and reviews of records, the allegations have been determined to be Substantiated. A finding of Substantiated means that the preponderance of evidence standard has been met. California Code of Regulations, Title 22, Division 12 Chapter 1 Article 07. Physical Environment, Section 101238 “Buildings and Grounds” and California Code of Regulations, Title 22, Division 12 Chapter 1 Article 06. Continuing Requirements, Section 101212 “Reporting Requirements" are being cited.

Please refer to LIC9099-D for documentation of deficiencies.

Notice of site visit was given and must remain posted for 30 days.

Exit interview conducted and report was reviewed with Larisa Nikogosyan, Director.
