On 3/12/2024 Licensing Program Analyst (LPA), Loyce Phillips conducted an unannounced Required-3 Year Inspection and was met by Licensee Mariza Mendoza. LPA observed 7 children and 1 infant present with Licensee and Assistant. Days and hours of operation are Monday through Friday 7:30am to 5:30pm. The facility does not have liability insurance, but waivers are on file.

LPA toured inside and outside the facility. This is a 1 story home with 3 bedrooms, 1 1/2 bathrooms, living room, dining area, kitchen, family room and detached garage. The home has an ADU on the property. LPA did not observed in child care activities conducted in the ADU. LPA reminded Licensee that no eating, toileting or napping can be conducted in the garage area.

The areas that are accessible to children: Bedroom #2, #3, living room, dining area, kitchen, family room, 1/2 bathroom (located in family room) garage (for outdoor activities only) and back yard for outdoor play.

The areas that are off limits to children: Master bedroom, bathroom located in the hallway and ADU.

There is no swimming pool or other bodies of water on the premises. There are no firearms or ammunition allowed or stored on the premises. No poisons were observed during the inspection. Detergents, cleaning compounds, medication and other hazardous items are made inaccessible to children.

There is a full charged fire extinguisher located in the kitchen. The facility has a working smoke detector, inside the home. The facility has adequate heating and ventilation for safety and comfort. Safe toys, materials and play equipment were observed indoors and outdoors. LPA observed First Aid kit located in day care room. Children nap on mats and infants sleep in cribs. The home has working telephone service and LPA confirmed the phone number is 310-695-6277. 809-C

Licensee ensures that children in care are supervised at all times and is aware children shall not be left in parked vehicles. Car seats are used for transportation purposes only and are not used for sleeping children. Capacity as specified on the license is being maintained.

Licensee’s Mandated Reporter Training was completed on 4/28/2021. Licensee’s pediatric CPR/First Aid was completed on 1/30/2022. Deficiencies cited (see LIC 809D.)

This facility does not provides Incidental Medical Services – IMS. For IMS information see PIN 22-02-CCP. Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Safe Sleep - LPA discussed the safe sleep regulations with Licensee, Mariza Mendoza and discussed the Child Care Licensing Safe Sleep webpage at:https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. LPA also discussed the following with Licensee:

All cribs shall be free from all loose articles and objects. Bumper pads shall not be used in the cribs. There shall be no objects hanging above or attached to the sides of the crib. Infants shall not be swaddle while in care. Infant's head shall not be covered while sleeping. Infants shall not be sleeping in car seats. Provider shall physically document every 15 minutes.

MyChildCarePlan.org – Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

Per Title 22, Division 12, Chapter 3, of the California Code of Regulations, deficiencies are cited and technical advisories discussed.

An exit interview was conducted, a copy of this report was read and provided to the Licensee. This report shall me made available to the public upon request. LIC 9213 Notice of Site Visit was provided and required to be posted for 30 days.
