25 | On 5/5/2022 at 10:30 am Licensing Program Analyst (LPA), Dalicia Adkins conducted an
unannounced Annual Required Inspection and was met by licensee, Dalia Schahaf. LPA Adkins disclosed the purpose of the visit and was granted entrance onto the facility. Licensee guided LPA on a tour of the home inside and outside. LPA Adkins observed 14 children, licensee and one child care assistant.
This is a two story home with 6 bedrooms and 5 bathroom. There is an activity room in the back yard. Days and hours of operation are Monday- Thursday from 8:00am to 4:00pm/ Friday 8:00am- 2:30pm.
LPA confirmed with licensee off limits rooms identified on facility sketch. All rooms identified as off limits were made inaccessible by locked doors. No swimming pool.
There are no firearms or ammunition on the premises. All poisons are kept in a locked storage area. No poisons were observed during the inspection. detergents, cleaning compounds, medication and other hazardous items are made inaccessible.
LPA observed one fireplace in the main living room, this fireplace is screened and inaccessible. There is a working fire extinguisher, smoke detector, carbon monoxide detector and adequate heating and ventilation for safety and comfort.
There are stairs in home, stairs has a barricade. Toys are safe and play equipment observed. The home has working telephone service and LPA confirmed the phone number is (310) 838-3733 and working cell number (310) 904-8732.
Licensee do not have any children under 12 months. LPA discussed Safe Sleep Regulations with licensee. |
