§1596.8662. Availability of information regarding detecting and reporting child abuse and neglect; training for mandated reporter who is licensed day care provider, administrator, or employee of a licensed child day care facility; proof of completion
(b) (1) On or before March 30, 2018, a person who, on January 1, 2018, is a licensed child care provider, administrator, or employee of a licensed child day care facility shall complete the mandated reporter training provided pursuant to paragraphs (2) and (3) of subdivision (a), and shall complete renewal mandated reporter training every year following the date on which he or she completed the initial mandated reporter training.
No deficiencies were cited during this inspection. Exit interview conducted with Licensee, Darnisha Shepherd. A copy of this report, LIC 857, LIC 859 and Notice of Site Visit were discussed and provided to Licensee. Licensee’s signature on this form acknowledges receipt of these rights.
Notice of Site Visit has been posted (LIC9213). The notice shall be posted for 30 consecutive days. Failure to maintain posting as required will result in a $100.00 civil penalty. Copies of this report must be posted for 30 days in visible location the authorized representatives of children.
LPA Johnson observed the weapon locked and stored in facility office. There are age appropriate toys. Age appropriate napping equipment (cots). The required Fire Extinguisher (2A10BC), Smoke Detector and Carbon Monoxide Detector are in operable condition. Fireplace is screened (Family Room). Home has central AC and heat. CPR/First Aid expire 06/2021. The First Aid kit was observed and is complete.
Requirements for fingerprint clearances and associations were discussed with the Licensee. The "Notification of Parent's Rights" poster must be posted in an area of the home accessible to parents.
Licensee was advised of the requirement to report unusual incidents and/or injuries to the parent/guardian and Licensing within the time frame specified by the regulation (call within 24 hours and submit the report within 7 days) and on the form LIC624B.
Licensee was advised that the Notice of Site Visit must be posted at the entrance of the facility for a period of 30 days. If this requirement is not met, civil penalties in the amount of $100 per violation will be assessed.
