Licensing Program Analyst (LPA), Keyona Scott, conducted an unannounced annual required inspection to the child care center on 09/28/2022. Upon arrival, LPA was directed to the location of the STAR Pre-K classroom by Castle Heights Elementary School office staff. At arrival to the STAR Pre-K classroom, LPA did not observe any children or staff present. LPA arrived back at the office of Castle Heights Elementary School and was directed to the location of the STAR Pre-K lunch area. As LPA was walking back to the STAR Pre-K area, LPA observed the STAR Pre-K children and staff lining up near the classroom. LPA observed proper teacher to children ratios. At 12:03 PM, LPA observed 19 children with two (2) teachers. LPA met with Director, Ana Cueller and was directed to the STAR Pre-K classroom. After Director prepared the children for nap time, LPA was guided on a tour inside and outside of the STAR Pre-K area. All adults, working or volunteering at facility have a criminal record clearance or exemption.

This is a preschool program that is located on-site at Castle Heights Elementary School. This STAR Pre-K program consists of one classroom and serves children ages four (4) to five (5) years old. LPA observed kitchen play sets, books, blocks, arts and crafts materials, board games, puzzles and other age appropriate toys and playthings in the STAR Pre-K classroom. LPA also observed electrical outlet safety covers, backpack hangers (on wall at classroom entrance), refrigerator, floor mats, table and chairs, and multiple starage cabinets in the STAR Pre-K classroom. Program operation hours are Monday through Friday, 8:15 AM to 2:15 PM and extended care hours are 2:15 PM to 6:00 PM.

Physical Plant
LPA observed facility sketch on file at El Segundo Child Care Regional Office (ESCCRO) and advised Director to provide an updated facility sketch of classroom and also an updated map of the Castle Heights Elementary School to the ESCCRO. The facility was kept clean, neat and orderly. STAR Pre-K program utilizes four bathrooms on school premises; two restrooms on the school's main play yard.

LPA observed restrooms on the main play yard and kinder yard as follows: boys restroom (three (3) sinks, four (4) urinals, two (2) toilets) and girls restroom (three (3) sinks, four (4) toilets) and two restrooms on the kinder yard: boys restroom (two (2) sinks, two (2) toilets, two (2) urinals and girls restroom (three (3) sinks, two (2) toilets. There is one (1) toilet and one (1) sink in the boys restroom on the main play yard that are out of order. LPA observed all other sinks and toilets to be safe, sanitary and functioning. All poisons and hazardous items are stored in the STAR Pre-K classroom in locked storage cabinets and are inaccessible to children in care. All floors are clean and safe. LPA observed the classroom to be clean and free of litter, rubbish and rodents and/or any other vermin. Trash cans used to discard food have tight fitting lids. Drinking water is readily available both indoors and outdoors. Facility was observed to be free of flies, gnats and other insects and rodents.

STAR Pre-K outdoor play yard is located at the North end of the school. Outdoor play equipment is in safe condition and is free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. LPA observed playhouse, kitchen play sets, slides and other age appropriate equipment. There is a shaded tent with benches near the play yard area that is utilized during meal and snack time.

FACILITY RECORDS
There is at least one person present at facility that is trained in Pediatric CPR and First Aid. LPA observed Director's Pediatric CPR/First Aid with an expiration date of 09/16/2023. Director completed Mandated Reporting training on 11/02/2021.

LPA reviewed two staff files of the following personnel records during today's inspection: Staff Qualifications; Proof of immunization of measles (MMR), pertussis (Tdap) and influenza; Current Pediatric CPR and First Aid Certification; TB clearance or risk assessment; LIC 503 (Health Screening Report); LIC 508 (Criminal Record Statement); LIC 9108 (Statement Acknowledging Requirement to Report Child Abuse); Mandated Reporter Training Certificate; LIC 9052 (Employee Rights)

LPA reviewed ten children's files of the following children's records: LIC 613A (Personal Rights); Admission Agreement; LIC 701 (Physician's Report); LIC 995 (Notification of Parents' Rights); LIC 627 (Consent for Emergency Medical Treatment); Immunization Record

The following was thoroughly discussed:
Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. The following information regarding ADA was provided: US Department of Justice

Director was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Failure to comply with posting requirements shall result in an immediate civil penalty of $100.

The facility was operating in substantial compliance during today’s inspection on 09/28/2022. Per Title 22, Division 12, Chapter 3, of the California Code of Regulations, no deficiencies cited.

A notice of site visit was given and must remain posted for 30 days.

Exit interview conducted and report was reviewed with Site Director of STAR Inc. School Age Child Care Program (facility number 191609770), Leigh Cohen in absence of Director Ana Cueller who needed to leave for the day.
