On 07/13/2023 at 8:30 a.m. Licensing Program Analyst (LPA), Doris Whitmore conducted an unannounced annual inspection. LPA met with site supervisor. Isabel Vitela LPA toured the facility inside and outside. There were no children in care. Children will return August 21, 2023. All center staff that was present during today’s visit had fingerprint cleared and associated to the designated license number.
LPA observed all posting requirements for operation on the posting board LIC 203A-License, LIC-9213-Notice of site visit, LIC 610 A-Emergency Disaster Plan, LIC 9148-Earthquake Preparedness Check List, PUB 394-Notification of parent’s rights poster, LIC 613A- Personal Rights, PUB 269- Child Car Seat Law, and Activity Schedule. There was no menu posted due to no class in session.
The premises has one Full Day Class from 8:15a.m. to 2:15p.m. and one State Pre-school Class form 8:00a.m. to 3:00p.m. Monday through Friday. LPA inspected the furniture and equipment for age appropriateness and good repair. Telephone service, heating, lighting, and ventilation were evaluated. Storage for children's belongings, isolation area was inspected. Children napping equipment are in good condition. Age-appropriate sinks and toilets were inspected for availability, good repair, water temperature, toilet paper, paper towels, area safety and sanitation. All classrooms have trash can with tight lids, first aid supplies, smoke detectors; carbon monoxide/fire extinguishers were observed. A review of medication policy, including administering, labeling, storage, and records was made. LPA reviewed the forms Medication to be taken during school hours, Individual Health Care Plan (IHP), Medication Checklist, Consent form for Nebulizer Care.

LPA was able to view 3 staff files and 10 children files. Children and staff records were reviewed and in good order. There is one staff that is currently has CPR and Fist Aid. LPA Whitmore reviewed the sign in sheet of training that took place 06/23/2023 for First Aid and CPR. Staff are currently waiting on the issue of cards.

During this inspection, LPA observed no physical changes to the facility. LPA observed a working telephone on the premises, heating, lighting, and ventilation were evaluated. Furniture and equipment were inspected for age appropriateness and good repair. Page 1

Napping equipment were observed to be clean and good condition. Adequate storage for children's belongings, bathrooms facilities, and separate area for isolation and care of ill children located in the office area. All Classrooms have igloos for drinking water. All trash cans had tight lids. Documentation of current Fire/Earthquake disaster drill was observed monthly. First Aid supplies were inventoried and available. Sign in/out were observed.

The facility serves breakfast, lunch, and snack and center Per center staff, foods and snacks were delivered daily to the site from the main kitchen in Long Beach. An inspection of cleaning and food supply storage areas was made. Food preparation area was toured for safety, cleanliness, adequately equipped and inaccessible to children in care. Food and snack items are properly stored. Outdoor area observed to be fenced. Climbing structure, slides and apparatus equipment securely anchored and has cushioned with material that absorbs falls zone. Bikes were observed in good condition. LPA observed other educational materials were available for children to utilize. Play area observed free of miscellaneous debris or hazards and inaccessibility to bodies of water. Required shade and rest areas are provided, drinking water readily available and accessible to children. LPA checked restrooms.


LPA reminded that it is the facility responsibility to know the regulations as well as anyone who assists in providing care. Site supervisor was advised on how to access quarterly reports, forms, and regulations for Child Care online at www.cdss.ca.gov. LPA also advised to read the Child Care quarterly updates every season as the come out to stay informed of any changes or updates to the regulations. LPA informed the Child Care Advocate Program (CCAP) provide many other helpful resources to the licensees and the public. Site Supervisor Isabel Vitela was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.Page2

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP). LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP

LPA discussed the safe sleep regulations with Site Supervisor Isabel Vitela and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep as an additional resource. LPA also informed licensee [or facility representative] of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of “medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. Site Supervisor Isabel Vitela was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California

A notice of site visit was given and must remain posted for 30 days.

Exit interview conducted and report was reviewed with the licensee Isabel Vitae. There were no deficiencies cited during this visit.
