On 8/16/2022 12:10PM, Licensing Program Analyst (LPA) Miranda conducted an announced case management inspection – POC at St. Sebastian Preschool, located at 1430 Federal Avenue, Los Angeles, CA, 90025 for the purpose of following up of the playground equipment located at the outdoor area of the facility. During this inspection, facility was closed due to summer break, and no children were observed in care. Per Director requested, LPA inspected the outdoor playground area as a plan of correction.

Based on the playground reported from CPSI Playground Inspection, an inspection was conducted by inspector David Christensen on 4/7/2022 for intended age of user for 2 years old to 5 years old. Per his report, corrections need to be completed at the playground equipment area to be in compliance for the health and safety of the children in care.

During this inspection, LPA inspected the outdoor playground area and Director was unable to provide a new report from CPSI Playground showing that facility is in compliance. Per director an update report written by CPSI Playground Inspection will be submit no later than 08/22/022, showing that all correction was completed. Director agreed to submit a copy of the updated reported. After, receive a copy of the report LPA will review and will contact Director regarding the playground area. Also, LPA observed planters and plants by the fence area with sharp points located at the playground area. Per Director she will remove all planters and plants. Photos will be submit to LPA Miranda via email no later than 08/22/2022.

Based on the inspection that was conducted today and from the last reported provided by Facility from CPSI Playground Inspection dated on 04/07/2022, facility shall maintain the playground area as an off-limit.

An exit interview was conducted, and a copy of this report along with notice of visit was given to Director.
