On 4/12/2023 Licensing Program Analyst (LPA) Judy Laureano conducted an unannounced Annual Required Inspection at St. Sebastian Pre-K located at 1430 Federal Avenue, Los Angeles, CA 90025 . LPA was greeted by Marisa Fuentes, facility director. LPA toured the facility both indoors and outdoors and observed 27 children and 3 staff members and director providing care and supervision.

Facility operates Monday through Friday 7:00 am to 6:00 p.m. Facility has a maximum capacity of 60 preschool age children, ages 2 years old to 5 years old. Facility currently has an outdoor waiver for the outdoor activity space: There shall be no more than 47 children outdoors a the same time. There shall be no child on the playground without direct care and supervision by qualified staff. The required 1:12 teacher to child or (1:15 teacher with aid to child) ratio shall always be maintained.

Facility is located within a private elementary school. Preschool program will only use preschool room 1, 2 and 3. Currently facility is only utilizing 2 preschool classrooms; classrooms 1 and 3.

There is no swimming pool or other bodies of water on the premises. There are no firearms or ammunition allowed or stored on the premises. Medications are made inaccessible and stored in the classroom that the child is enrolled with all the necessary LIC forms. First Aid kits were observed in each classroom.

Teachers do the day to day cleaning in the classroom. Disinfectants, cleaning solutions and other hazardous items are made inaccessible in locked bottom sink cabinet in each classroom. Facility has a designated person, that cleans and disinfects the classrooms every evening.

The outdoor area was observed and inspected. Director informed LPA that currently children will not be using the back end of the outdoor space, Climbing Structure area. Facility noticed a large beehive by the shared fence and are attempting to work with neighbor, private property, and animal control to have the large beehive removed. LPA instructed director to complete an unusual incident report. LPA also instructed director to email an updated outdoor play schedule to LPA by 4/21/2023, reflecting the use of only the 2 front sections of the play yard. LPA reminded director to contact LPA when issue has been resolved. Climbing structure area WILL NOT be in use until further notice.

Furniture and equipment are in good condition, free of sharp, loose or pointed parts. Playground equipment is in safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. Outdoor area provided a shaded area for children to play. Areas around high climbing equipment and slides have cushioning material to absorb falls.

All toilets and handwashing facilities are in safe and sanitary operating condition. Floors in the facility are clean and safe. All kitchen, food preparation and storage areas are clean, free of litter/rubbish and free of rodents/vermin. Facility provides occasional snack and menu is posted outside the classrooms on the parent board. Families are responsible for bringing meals and snacks in appropriate storage containers such as lunch bags with ice packs and/or thermos. All food is protected against contamination and any contaminated food is discarded immediately. Drinking water is available both indoors and outdoors. Children bring their own refillable water bottles and staff refills water as needed. LPA observed water dispensers in each classroom.

Solid waste storage containers have tight-fitting covers and are in good repair. The facility is free of flies, insects and rodents. Facility contracts with a Pest control vendor to ensure that facility stays pest free. LPA observed working fire extinguishers in each classroom. Facility has a centralized fire alarm and sprinkler system throughout the facility.

Prior to working or volunteering in a licensed childcare facility, all individuals subject to a criminal record review have received a criminal record clearance or exemption. Upon notification from the Department, the licensee will comply and act immediately to terminate the employment of, remove from the facility or bar from entering the facility for any person it is deemed necessary while the Department considers granting or denying an exemption.
LPA reviewed 5 staff files and observed 4 files did not have current Mandated Reporter Provider Training certificate for LPA’s review. LPA issued a technical violation. At least one person trained in CPR and Pediatric First Aid is present when children are at the facility or at offsite activities. LPA provided director with the list of approved EMSA approved vendors to use as a resource when planning future training.
LPA reviewed a sample of children’s files, 8 files, and observed files were complete. LPA provided facility with a copy of the LIC 311A and LIC 125 to use as a reference to ensure all files have current documents.
The name of the child care center director or fully-qualified teacher(s) designated to act in the director’s absence has been reported to the Department. The person who signs the child in/out of the facility shall use their full legal signature and record the time of day. LPA observed sign in and out sheets outside the preschool classrooms.
All children are under supervision, including visual supervision, of a teacher at all times. Facility maintains a ratio of one teacher supervising no more than 12 children in care.

Facility representative, Marissa Fuentes was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.
Incidental Medical Services (IMS) currently IS being provided. Licensee is aware that an IMS plan is required to be submitted to the licensing office if they provide any of these services. Information regarding Americans with Disability Act (ADA) can be obtained by contacting US Department of Justice toll free ADA Information line at (800) 514-0301(voice), (800) 514-0383 (TDD) and website link https://www.ada.gov/childqanda.htm.

LPA and facility admin discussed the Community Care Licensing website www.ccld.ca.gov which will provide access to Provider Information Notices (PINs), Quarterly Updates, COVID-19 Information and Resources, Mandated Reporter Training, Safe Sleep in Child Care, Lead Poisoning Facts, Forms and Regulations.
A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
