On 12/10/2024 Licensing Program Analyst (LPA) Tyra Chavies, conducted an unannounced  3 Year Required visit at Sound of Music Preschool. LPA met with Director Lisa Garcia and toured the facility indoors and outdoors. Days and hours of operation are Monday through Friday 7:00 AM to 6:00 PM. LPA Chavies confirmed the phone number is: (323) 733-1327. There were 18 children in care being supervised by 3 Staff members. LPA Chavies informed director that the cook can not supervise until he/she has the minimum qualifications. PM snacks are provided by the center as well as breakfast and lunch. There is currently no Incidental Medical Services (IMS) are been given at this time.

There are no bodies of water on the premises. There are no firearms or ammunition allowed or stored on the premises. Disinfectants, cleaning solutions, medication and other hazardous items are made inaccessible. All poisons are kept in a locked storage area.

Classroom 2 and 3 are currently being used for child care. Classrooms 1 and 4 had no children in the space during the visit. This facility does have 3 different playgrounds. The front playground and the playground on the side of classroom 3 are currently being used. Indoor furniture and equipment are in good condition, free of sharp, loose and/or pointed parts. The facility floors, bathroom and classrooms meet Title 22 Health and Safety regulations. LPA observed cots for sleeping, age appropriate toys and books in primary care area. The surface of the outdoor activity space is maintained in good condition. During the inspection of the outside front play structure, LPA Chavies noticed several holes (size of an adult foot) in the material to absorb falls. LPA Chavies advised director that the front playground material to absorb falls must be replaced/or repaired.

LPA inspected and verified that there is a fire system connected throughout the facility. The facility does have a fire extinguisher which has an inspection date of on January 22,2024 on the premises. Facility has one or more functioning carbon monoxide detectors. LPA advised director of the LIC 9148 (Earthquake preparedness form). The facility must adhere to the regulations in section A, B and C. No disaster kit was observed during visit. Drinking water is available both indoors and outdoors upon request. Lead testing has not been done. Director has informed LPA Chavies, that the facility only uses bottled water for drinking water and to prepare meals.

POSTING REQUIREMENTS: License and other relevant notices are visible for public view and correctly posted on the wall (Facility Sketch, PUB 394, LIC 9148, LIC 610A). Fire and disaster drills are being conducted as scheduled every six months.

CHILDREN'S RECORDS: LPA Chavies reviewed 6 children's files. All files are up to date. One or more files are missing immunization records. 

STAFF'S RECORDS: LPA Chavies reviewed 4 staff files. Staff records must be updated. One or more staff members have expired Mandated Reporter Training, missing immunization, Missing LIC 9052 (employee rights) and education documents. Director has taken a EMSA Pediatric First Aid and CPR with an expiration 09/2026.

The following were also discussed with the director:
  • Individuals who are 18 years of age must obtain a criminal record clearance.  Individuals within one month of their 18th birthday must be fingerprinted immediately. Failure to obtain a criminal record background check clearances prior to initial presence in the facility will result in an immediate $500.00 dollar civil penalty and $100.00 more per day until licensee is in compliance 
  •  Commencing September 1st, 2016, prohibits a person from being employed or volunteering at a childcare facility or family day care if he or she has not been immunized against influenza, pertussis and measles.
  • New Immunization Requirement: Law enacted by SB 277, beginning January 1st, 2016, personal beliefs exemptions will no longer be an option for the vaccines that are currently required for entry into childcare or school in California. Personal beliefs exemptions already on file will remain valid until the child reaches the next immunization checkpoint.
  • New Appeal Process: A licensee may file an appeal, in writing 15 business days from the date of receiving the penalty assessment.
When regulations are approved/changed/updated, providers will be notified through Provider Information Notice (PIN). 

Per Title 22, of the California Code of Regulations, the following Type A, Type B and Technical Violations were discussed. (See LIC 809 D and LIC 9102)

An exit interview was conducted with Director, Lisa Garcia.
A copy of this report was read and given to Director as well as LIC 9213 (Notice of Site Visit form.) LIC 9213 is required to be posted for 30 days.

  • Lead Testing- Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test.
  • For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP).

When regulations are approved/changed/updated, providers will be notified through Provider Information Notice (PIN).
