An unannounced Annual Random Inspection was conducted by Licensing Program Analyst (LPA) Shandra Powell LPA met with licensee Alicia Monteza, Licensee who guided LPA on a tour of the facility. This is a two story home consists of 5 bedrooms, 3 bathrooms, kitchen, family room/living room and a detached garage. There is a single dwelling home on the same grounds of the facility that sits behind home behind the play area has a gate that is locked at all times. (Licensee Mother, Father and Brother reside in back house) Child care is conducted in the family living room, first bed room (infant napping room) and dining room areas. Children use the bathroom located in the dining room area only. Children do have not have access to the second bedroom and adult bathroom on the first level which is off of the living room. Off limit rooms have child proof locks on door handles. Licensee acknowledged that children may never enter these off-limit areas. The entrance leading to the upstairs area of the home has a door that remains locked. Per licensee children play in the backyard. LPA observed it to be fenced in, safe and clean. Per licensee the garage door stays lock at all times during operating hours. LPA did not observed any bodies of water in the home during this visit. Currently residing in the home are Licensee, licensee aunt and licensees two children. Home Present at the time of inspection was licensee, aunt (employee) and licensee mother.


Upon arrival LPA observed 1 infant, 5 preschoolers and 1 school age child. Licensee states that there are no weapons or firearms on the premises. The backyard is adequately fenced. There are age appropriate toys and equipment on the premises. The smoke detectors and carbon monoxide detectors are present in the home and are operational. Fire extinguisher is present in the home. Licensee prepares breakfast, lunch and snack for children in care. Home was clean and orderly with heating and ventilation (central). Cleaning compounds inaccessible by safety locks on kitchen cabinets.

During file review LPA did observe immunization records complete for licensee and employees.

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Family Child Care Homes Section 102417. —CPR Card valid until: 08/2021
—Child Care Roster, Disaster Plan, Fire Drill conducted on June 2019 (Earthquake Drill) and Children's Records were reviewed and discussed.
—Children records and required licensing forms were discussed as well as mandated child abuse reporting and criminal record clearance requirement. All children files must contain a LIC 700 form for each child.

In the absence of the Licensee, a qualified adult must be present supervising the children; a qualified adult is an individual who has a valid and current adult/infant CPR & Pediatric First Aid certification and a valid criminal record clearance associated to the facility license.

A current roster of children enrolled must be available and maintained for a period of three years, even after children no longer are attending the facility. Annual fees must be paid promptly and by the due date or a late fee shall be assessed and/or the License shall be terminated.

The fire extinguisher type 2A-10BC must be serviced annually or as often as necessary. Smoke and Carbon Monoxide detectors should be checked and batteries replaced as needed. Changes should be reported to the Department as soon as they occur such as construction, remodeling, telephone number changes and/or if you move from your home.

Any unusual incidents or injuries must be reported to the Department within 24 hours via telephone and within seven (7) days in writing. Fire and safety drills must be performed every six months and documented for review by the Department. Smoking is prohibited in a family child care home. Children and Staff records must be maintained and updated as needed and must be available for review by the Department.

All adults living and working in the home shall be made of aware of the Departments right to inspection authority, which includes but not limited to the right to enter the home when children are being cared for, interview children and adults and review documentation.Baby walkers, saucer chairs, bouncers or any similar items are prohibited. Sudden Infant Death Syndrome (SIDS) and Never-Shake-a-Baby were discussed. LPA provided the licensee with a copy of the Child Care Provider’s Guide to Safe Sleep, by American Academy of Pediatrics and the New Proposed Safe Sleep Regulations and Best Practices, Helping you to reduce the risk of SIDS. LPA also consulted and explained Child Abuse Reporting, Updated Parent’s Rights Poster with Complaint Hotline information.

· Dog(s) and/or pets should be isolated from children in care.
· It is recommended that First-Aid kits be available on premises.
· Outdoor supervision required at all times.

INFORMATION TO BE POSTED IN YOU FAMILY CHILD CARE HOME – You are required by Law to post the following:
Facility License (LIC 203), Emergency Disaster Plan (LIC 610A), Earthquake Preparedness Checklist (LIC 9148), Notification of Parent’s Rights (PUB 394). A Notice of Site Visit (LIC 9213): Must be posted for 30 days after each site inspection by a Licensing Representative. Any Licensing Report documenting a Type “A” deficiency must be posted for 30 days during the hours that children are in care. Any Licensing Report or other document verifying compliance or non-compliance with the Department’s order to correct a Type “A” deficiency must be posted for 30 days during the hours that children are in care. These forms may also be downloaded from our website: www.ccld.ca.gov

Based on the LPA observations and in accordance with California Code of Regulations Title 22. No Deficiencies are being cited during this inspection.

The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit made by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.

Exit interview was conducted with Alicia Monteza, Licensee, including, but not limited to Appeal Procedures, Site Visit and Initial Appeal Rights.
