25 | Licensing Program Analyst (LPA) Keyona Scott, conducted an unannounced Case Management inspection to the Gardena Early Education Center, on 10/03/2019, for the purpose of following up on an unusual incident report submitted to the Regional Office on 09/30/2019. The incident involved two, three year old siblings, that were found outside of the facility.
LPA met with the facility's Principal, Desilisa Mitchell, and toured the facility. There were 112 preschoolers in care, with the adequate teacher/adult to child ratios, at the time of the facility tour.
During today's inspection, the physical plant was inspected, interviews were conducted, children's files reviewed and observation conducted. The following documents were also obtained:
- Child Care Facility Roster (LIC 9040)
- Sign In/Sign Out Sheet for September 2019
- Staff Schedule for July 1, 2019 to July 30, 2020
Based on todays interview and observation, a citation was issued for Lack of Care and Supervision.
During this inspection, the facility was cited for Type A, Title 22 Deficiency (see LIC 809-D attached).
The following was thoroughly discussed with the Principal:
Principal was notified that a copy of this Licensing Report must be given to each existing parent by the end of today or next day children are in care, and to the parent of children enrolled over the next 12 months. In addition, a copy of the Acknowledgement of Receipt of Licensing Reports (LIC 9224) must be signed by each parent and kept in each child's file.Page 1 |
