On 2/14/2023, Licensing Program Analyst (LPA) Suzette Ornelas conducted an unannounced case management visit for the purpose of investigating the Unusual Incident Report that was self reported via telephone on 1/18/2023. LPA Suzette met with Lead Teacher Gloria Duban who assisted LPA with indoors and outdoors facility tour. Days and hours of operation are Monday through Friday 7a to 5:30p.

LPA observed all required postings in a prominent area. A review of the sign in/out sheet was conducted to verify the current census of children. LPA observed 69 children and 25 adults. All children are under supervision, including visual supervision, of a teacher at all times. Capacity and limitations as specified on the license are being maintained. A walk through of the classroom area was conducted. LPA observed 7 licensed preschool classrooms on the premises.

All areas identified on the Facility Sketch were inspected. The following staff were present during this inspection:



RM 1: 7 children with a ratio of 3 teachers
RM 2: 6 children with a ratio of 3 teachers
RM 3: 12 children with a ratio of 3 teachers
RM 4: 10 children with a ratio of 3 teachers
RM 5: 6 children with a ratio of 3 teachers
RM 6: 14 children with a ratio of 6 teachers
RM 7: 10 children with a ratio of 2 teachers

LPA then met with Principal Desilisa Mitchell who assisted LPA with obtaining the following documents:

-Children's Roster (licensing form) .
-Copy of SCAR .
-Children's Sign In/Out Sheets for the month of November
-Daily Routine Schedule for all Classrooms
-Staff Work Schedule .
-Verification of November Classroom 2 staff

Exit interview was conducted.

This report shall be made available to the public upon request.
