On 07/28/2022 Licensing Program Analyst (LPA) Doris Whitmore met with the Director Erin Tamashiro for the purpose of an unannounced inspection. LPA Whitmore toured the facility, all identified rooms per facility sketch were inspected, the following was observed. Hours of Operation are Monday through Friday 7:15 a.m. – 5:30p.m. At the time of Inspection there were a total of forty-seven children, five teachers, and one director. LPA toured the facility inside and outside.

LPA observed an upstairs and downstairs for the facility. Entrance to the preschool can be accessed through main entrance (door located adjacent to parking lot). Classrooms are located behind locked doors for safety. School office is located at the top of the stairs. Upon arriving at the preschool LPA observed children in the classrooms and parents dropping off. LPA observed teachers supervising the children in the classrooms upstairs and downstairs. LPA observed one ipad (located at downstairs entry). Parents sign children in and out by using the Pro- Care App on their phones or scanning QR code. A walk through of each classroom was conducted, classrooms were found to be clean and free from any potential hazards. LPA observed classrooms to have dramatic play, blocks, arts& crafts, manipulatives, & books.

Furniture in each classroom was found to be in good repair and age appropriate. There is adequate heating, lighting and ventilation. There is a working fire extinguisher, smoke detector and carbon monoxide. Drinking water is provided or the child can bring their own water. Water is accessible for the children in each classroom. Director states that the isolation area for sick students is located in the Director's office on a cot or mat. LPA observed 2 restrooms for children in upstairs classroom and 4 restrooms for children in downstairs classroom. The restrooms area(s) were inspected, there are sufficient toilets and sinks to accommodate the facility capacity. Toilets flush properly, toilet and sinks are reachable by the children in care. Each restroom has adequate toilet paper and paper towels available. The school provides two snacks and an optional lunch. LPA observed a separate kitchen next to the break room. Kitchen is inaccessible to children in care. All snacks are stored in tight containers and stored in separate storage area in director’s office in a cabinet
Inspection of the outdoor play area was conducted climbing structures, slides and other large play equipment is found in good repair.

Kitchen is inaccessible to children in care. All snacks are stored in tight containers and stored in separate storage area in director’s office in a cabinet
Inspection of the outdoor play area was conducted climbing structures, slides and other large play equipment is found in good repair. Drinking water is readily available on the play yard. There is adequate shade for the children in care. Fencing around the perimeter of the play area is at least 4 feet high. Playground is free from miscellaneous debris. Playground can be accessed from downstairs classroom. First Aid supplies are stored in each classroom.
LPA reviewed a total of 10 children’s files that were completed with the required documents. LPA reviewed a total of five teacher files and 1 file for the director all required documentation were in the files. Mandated Reporter Training Certificates were all current date of competition 07/23/21. Director is fully aware that the Mandated Reporter Training is only good for two years. LPA reviewed all First Aid Cards with the expiration date of 10/23/2023.


The following was discussed with the licensee:
Mandatory Forms for the children’s files and provider’s files, Requirements for fire drills, earthquake drills and documentation for both. Role and responsibilities of being a mandated reporter were discussed. The licensee was advised how to access forms and Regulations for Family Child Care online at www.ccld.ca.gov . Licensee was made aware that it is their responsibility to know the regulations as well as anyone who assists in providing care. Licensee was advised on how to access quarterly reports, forms, and regulations for Child Care online at www.ccld.ca.gov. The licensee was advised that inaccessibility of hazards must be constantly reassessed depending on the children in care. Licensing must have the facility’s phone number at all times; if the phone number is changed, licensing must be notified. Regulation prohibits the smoking of tobacco on the premises.

Children and staff records were reviewed for completeness records were found to be complete.
Director advised that records for all children and staff must be maintained for 3 years after separation from the facility. A record of teacher’s names was check against current facility roster. LPA shared information with the Director on How to access Guardian?

Requirements for fingerprint clearances and associations were discussed with the licensee. Licensee can be cited a civil penalty of $100 per day, up to $500.00 (5 days) for the 1st offense and up to $3000.00 for the 2nd offense within a 12-month period. The licensee was advised of the requirement to report unusual incidents and/or injuries.

The licensee was advised that the Notice of Site Visit must be posted at the entrance of the facility for a period of 30 days. If a serious violation is cited, a copy of the licensing report (LIC809 or LIC9099) must also be posted for 30 days. If these requirements are not met, civil penalties in the amount of $100 per violation will be assessed. Copies of the reports must also be provided to each parent when a serious deficiency, Type A, is cited (LIC9224).

The licensee was informed of The Child Care Advocate Program (CCAP) that is administered from within the Community Care Licensing Division. CCAP participates in many community activities and special projects in order to disseminate information on the State’s licensing role, provide information to the public and parents on childcare licensing, and provide many other helpful resources to the licensees and the public. AB 1207: Beginning on January 1, 2018, this law requires all licensed providers, applicants, directors and employees

Senate Bill 792: this bill, commencing September 1, 2016, prohibits a person from being employed or volunteering at a childcare facility or family day care if he or she has not been immunized against influenza, pertussis and measles.
