Licensing Program Analysts (LPAs) Linda Voong and Franchesca White arrived at the above facility for the purpose of an unannounced 3yr - Required Annual Inspection. LPA Linda Voong announced the purpose of the visit and was granted entry into the facility by Licensee Jermaine Thompson. The hours of operation are 6:00 a.m. to 11:00 p.m. (Monday - Friday). There are 31 children enrolled, and 3 children present at the time of inspection. The ages of the children served in this facility are infant to 12 years of age. All adults in the home were discussed and have current criminal background clearance. The following documents were observed on the Parent Board: License, Parents Rights, And Earthquake Preparedness Checklist. The following documents were observed during the inspection: Emergency Disaster Plan, and The Disaster Drill Log with the last drill conducted May 5, 2025. The facility roster was available at the time of inspection and was current. Licensee provided

All life saving devices were observed. The smoke detector was operable and located in the office in the rear of the home. The carbon monoxide detector was not operable at the time of inspection. Licensee purchased a new detector. LPAs observed Licensee make the purchase. The 2A 10 BC fire extinguisher was observed to be fully charged and have a service tag dated January 23, 2025. The first aid kit is kept in the off limits office in the rear of the home, and was inventoried for ample supplies.

This is a single story, 3 bedroom/2 bath home. The main childcare areas are the living room, dining room and one (1) bedrooms, by the front of home. Other areas accessible to children are: 2 bathrooms, backyard, the Kitchen is accessible only as a walk way to the backyard and rear bathroom. Off limit areas, per Licensee and facility sketch are:

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

two (two) bedroom toward the rear of home, maintained off limits by a combination key lock. Bedrooms observed closed and locked during inspection. The garage in the backyard is maintained inaccessible by pad lock. All areas identified on the facility sketch that are accessible for children to use were inspected for safety, comfort, and cleanliness.

All areas used by children were inspected for safety, comfort, cleanliness, telephone service (cell phone), ventilation and heating (central air and heat).

At 10:00 a.m., LPAs observed the main care room to have a couch and three cribs, cubbies for the children's belongings blocking the built - in fireplace. There is a mounted flat screen tv. There is an air-conditioning unit mounted to the wall. There are three child size tables in the room with chairs stacked in the corner in front of the book case with children's books, and activities stacked on the shelves. To the left of the entrance is a room that is used for the children's play area. There is a baby gate for the safety of the small day care children who use this space. LPAs observed the room to have wall storage for the children's diapers, and personal items mounted to the wall. There is a dramatic play kitchen against the wall.

At 10:15 a.m. LPA's observed the kitchen to be made off limits with the use of a baby gate. Kitchen is used as a walkway to the play yard located in the rear of the home. Knives are stored high in a locked cabinet making them inaccessible to children in care. Licensee states that medications are kept in a locked container making them inaccessible to children in care

At 10:30 a.m., LPAs observed bathroom to have one sink, one toilet, hand washing items, and toilet paper. LPA's observed the floors to be free of debris, and slipping hazards. All poisons were removed from the bathroom and placed in an off limits area making them inaccessible to children in care.

At 10:45 a.m., LPAs observed the outdoor play area to have a shaded area with lunch benches for children to sit. There are trikes, a basketball court and age appropriate toys for the children. There is a slide and climbing set for day care children with soft floor padding. LPAs observed the perimeter of the play area to be completely fenced in for the safety of the children.

LPAs did not observe any bodies of water. Licensee states there are no pets. LPAs did not observe any pets. Licensee states that there are no firearms or weapons at the facility. LPAs did not observe any weapons or firearms during the inspection.

Licensee states that he will add a child safety lock to the off limits office.

At 11:15 a.m., LPA Voong reviewed the children's files to have all department documents. There are signatures needed on LIC700, LIC 627, and LIC 995A. Licensee states proof of completion of all documentation will be provided on or before June 5, 2025.

At 11:30 a.m., LPA Voong reviewed the staff files to have all department required documents. Jermaine Thompson has mandated reporter training certification 2/15/2026 (expiration date), CPR/1st Aid expiration date 1/21/2026. Tommy Thompson has mandated reporter training certification with expiration date of 2/15/2026, and CPR/1st Aid with expiration date of 1/21/2026. Licensee states that he will provide proof of immunization on or before June 5, 2025.

The following were discussed:
Licensee Jermaine Thompson was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

LPA discussed the safe sleep regulations with licensee Jermaine Thompson and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee Jermaine Thompson of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Licensee Jermaine Thompson was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

During the exit interview, the LICENSEE Jermaine Thompson , confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS.

Based on today's inspection, observations, interviews, and record review there will be no deficiencies cited in accordance with California Title 22 regulations.

A notice of site visit was given and must remain posted for 30 days.
