On 09/05/2024 at 9:30 AM an unannounced Annual/Random inspection was conducted by Licensing Program Analyst (LPA) Katrina Chicote. LPA announced purpose of visit and initially met with Jose Delgadillo, Assistant Principal. Facility is currently licensed for a capacity of 22, and per FR have 12 currently enrolled. The program currently operates Monday thru Friday from 7:30 AM to 2:20 PM. LPA was taken on a guided tour of the facility of both indoors and outdoors. Required postings were observed. Facility Representative (FR), Osvaldo Jimenez, Principal, joined after tour. LIC 125 was provided.

The facility is located in Room 13 of Corona Elementary School campus. At the initial start of the site visit, there is a total census of 12 children and a total census of two staff (S1 and S2) present.

Beginning at 9:35 AM, furniture and equipment were inspected for age appropriateness and good repair. LPA observed surface of classrooms to be safe and clean, and child size tables and chairs available for children with rounded edges and well maintained. LPA observed learning stations for dramatic play, math, and science with a variety of age appropriate toys maintained free of loose and sharp parts and a library area available for quiet play. Telephone service, heating, lighting and ventilation were evaluated. There were cubbies observed as storage for children's belongings. Cots were inspected and observed stored in classroom covered with linens stored separately in bags. Per FR linens are provided by the facility and washed by weekly laundry service. Availability of drinking water was reviewed. LPA observed children's personal water bottles available for drinking water. LPA observed drinking fountains in classroom that FR states have been shut off. Age appropriate sinks and toilets were inspected for availability, good repair, water temperature, toilet paper, area safety and sanitation. LPA observed two bathroom available in hallway right next to classroom that have ample sinks and toilets for capacity. FR and LPA discussed bathroom procedures at time of inspection. Hazardous items including poisonous cleaning compounds were stored inaccessible to children. Per S1, these items are not stored in classroom at time of inspection.

A first aid kit is kept on high shelf in classroom, which was inventoried for necessary supplies. LPA discussed requirement of having operable carbon monoxide detector present at the facility. LPA observed Assistant Principal call Plant Manager to discuss if carbon monoxide detector is hardwired. Plant Manager states he believes it could be but could not verify at time of inspection. Assistant Principal states that they will verify this information or obtain a physical carbon monoxide detector for the classroom. LPA observed smoke detectors and fire extinguishers are present in the facility. Disaster drill log was available and facility has conducted many drills, one of which was conducted on 09/05/2024. Isolation area for sick children is in school's Nurse's Office. Per S1, they currently have no children that require medicines.

Snack/lunch menus were reviewed and observed posted. Facility provides breakfast and lunch. Food prep areas not toured as food is delivered from Cafeteria. Trash cans used to discard food have tight fitting lids.

Beginning at 10:00 AM, outdoor equipment was inspected for safety, cushioning material, good repair and age-appropriateness. LPA observed surface of outdoor play areas to be safe and maintained for activities planned. LPA observed a variety of age appropriate toys available such as plastic play houses and large outdoor manipulatives. Required shade and fencing were inspected. LPA observed fixed shaded areas with benches and tables. LPA advised that the children need to be within the direct care and supervision, including visual supervision of the teacher(s) at all times. The children have access to to their water bottles during outside play. Play area was inspected for hazards and inaccessibility to bodies of water; no bodies of water or hazards observed.

Teacher-child ratios were observed and staff names recorded. Care and supervision was evaluated to determine if the basic needs of children are met and appropriate. Sign-in and out sheets and procedures were reviewed. LPA observed nine of 12 children present signed in at time of inspection. S1 provided clarification and states that parents incorrectly signed this week due to Monday being a holiday but all children present are signed in at time of inspection. Personal Rights of children were observed by LPA.

Staff and children’s records were reviewed for completeness. Inspection of required forms was made and documented on the LIC 857 for children and the LIC 859 for staff. LPA did not review confidential staff files during the inspection due to the staff confidential files being stored at the administrative office. CPR card expires on 06/2026 for S1. Staff present did not have proof of the AB 1207 Mandated Reporter Training

certificate on file. FR states they completed a Mandated Reporter training per school requirements and LPA provided technical support on which training was required for Licensing. All staff and employees have obtained a criminal record clearance or criminal record exemption as a condition of employment with the Los Angeles Unified School District (LAUSD).

The following were discussed:
LPA advised of the requirement to report Unusual Incidents and/or injuries to the parent/guardian and to CCL within the time frame specified by the regulation, this includes any changes as soon as they occur such as construction, remodeling, telephone number changes and/or if you move from your location. Copy of LIC 624 was provided and email for incidents were provided.

101200 (c) Inspection Authority of the Department
The Department has the authority to inspect, audit, and copy child or child care center records upon demand during normal business hours. Records may be removed if necessary for copying. Removal of records shall be subject to the requirements in Sections 101217(c) and 101221(d).


LPA advised the licensee to access forms, regulations and quarterly updates on the Child Care Licensing website at: www.ccld.ca.gov.

CCC COMPLETED TESTING AND IS IN THE PROCESS OF REMEDIATING LEAD EXCEEDANCES:
Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. LPA referred FR to the Department website for lead: Lead Toxicity Prevention and Water Testing Information.

MyChildCarePlan.org – Centers and Family Child Care Homes Licensee [or facility representative] was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

The facility was found in compliance per Title 22 regulations, there will be no deficiencies cited only advisories given today, 09/05/2024.

A notice of site visit was given and must remain posted for 30 days.
