Licensing Program Analyst (LPA) Alicia Bailey conducted an unannounced required annual inspection on 07/19/2021 at 2:00pm. Upon arrival, LPA Bailey met with Director Gwendolyn Crews. LPA was guided on a tour of the facility including both indoor and outdoor areas. This is an infant program licensed for (12) infants (ages 0-2 years of age). LPA Bailey observed two infants today during the inspection. During the tour, LPA observed that there was a preschool program (197413346) located on the premises that is physically separate from the infant center both indoors and outdoors.

All areas identified on the Facility Sketch were inspected. Upon arrival, the following staff were present during this inspection: Staff #1 with 2 infants Staff # 2 preschoolers six (6 preschoolers). The facility was observed to be within the license capacity and limitations. Facility operation hours are Monday to Friday from 7:30 AM to 4:00 PM.

At 2:15pm LPA Bailey observed the Infant program consists of 2 classrooms. Classroom 1 # ( Babyland) is for activities and eating . Classroom # 2 consist of the sleeping area.

Cribs and napping equipment were observed in the classrooms. LPA Bailey observed two infants in the activities room playing with age appropriate toys on the mats on the floor. Per Director Crews, the facility provides the of linen and bedding which are washed weekly. The center has additional sets in order to provide clean bedding daily. At this time, the office is used as an isolation area.

Infant Needs and Services Plan: The written infant needs and services plan shall be updated at least quarterly, or as often as necessary to assure its accuracy. Infant Care: LPA advised the applicant to sleep infants where they can be directly supervised at all time. LPA advised against sleeping infants in a separate room. LPA reviewed SIDs, Never Shake A Baby, and safe sleeping practices. Infants should sleep mouth up, on their backs, free of clutter surrounding their sleeping space. Safe sleep concepts were provided. LPA discussed and emailed a copy of PIN 20-24-CCP

LPA observed the furniture and equipment were inspected for age appropriateness. All changing tables and feeding chairs are clean and in operational condition. There is a sink with hot water within arm’s reach of each of the changing table located in each classroom. Indoor classroom was inspected to ensure that the floors have a surface that is safe and clean. Infants used personal drinking bottles for water and were observed to be properly labelled.

Disinfectants, cleaning solutions and other items that are dangerous to children, were inaccessible to children and are kept under the kitchen sink and is locked. Carbon monoxide detectors were observed and are operable.

At 2:30pm LPA Bailey observed kitchen areas/food preparation areas and food storage areas are kept clean and are free of litter, rubbish, rodents, and/or any other vermin. All storage containers for solid waste, including moveable bins shall have tight-fitting covers that are kept on, and in good repair. Trash cans used to discard food have tight fitting lids or solid waste bags shall be discarded immediately after each meal. Director Crews stated, all food is provided by parents and were observed to be properly labelled.

At 2:40pm LPA Bailey toured the Outdoor playground. The equipment is in good condition, free of sharp, or pointed parts. The surface of the outdoor activity space is maintained in a safe and is free of hazards. All areas around or under climbing equipment and similar equipment cushioned with material to absorbs a fall. There is adequate shade provided in the play yard. There are no pools or bodies of water at the facility. There are no firearms stored on the premises.

At 2:50pm the First Aid supplies were inventoried for quantity and expiration dates. There is are First Aid Kit in the office, and one in each classroom. A review of medication policy, including administering,


labeling, storage, and records was made. There are currently no children on medication.

All posting requirements were observed on the Parent Board, which is located in the office, as well as the board directly facing the office exit ,and in each classroom, there are some of the posting requirements. The required postings are: the license, Personal Rights, Child Passenger Safety Law, Notification of Parents Rights, and the snack and lunch menus. Sign in and out sheets were reviewed. An Emergency Disaster Drill log is kept. The last fire & earthquake drill was ran 02/10/2021 LPA Bailey advised Director Crews to conducted this week with children. Director Crews stated she will conducted a drill on 07/20/2021.

Staff and children records were reviewed for completeness including but not limited to Criminal Record Clearances for adults, Director Qualifications and verification of CPR/First Aid and health preventive practices documentation. Children and Staff files were reviewed for required forms.

Transportation: This facility does not provide transportation.

There were no deficiencies cited during today’s inspection. The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. Exit interview was conducted with Gwendolyn Crews Director, including, but not limited to Provider Rights, Appeal Procedures and Agencies Consultative Role.
