On 01/22/2024, Licensing Program Analyst (LPA) Cristina Castellanos conducted an unannounced Annual Required Inspection at the above-mentioned address. Licensing Program Analyst Castellanos met with Site Supervisor, Dimuthu Ratnasekare and toured the facility both indoors and outdoors. LPA observed 10 infants being supervised by 3 fingerprinted cleared staff members during today’s inspection. The facility is open Monday through Friday from 7:00am to 6:00pm. At approximately 11:00am Director Shirani Perera arrived at the center and met with LPA Castellanos and later left at approximately 12:30pm. Present during today’s inspection was Site Supervisor Dimuthu Ratnasekare.

Capacity and limitations as specified on the license are being maintained.

All children are under supervision, including visual supervision, of a teacher at all times. The facility is in compliance with Title 22 regulations regarding staff to infant ratios. Parents sign their child in/out of the facility. LPA Castellanos advised administrators to ensure that parents sign using their full legal name and the time of day.

There is no swimming pool or other bodies of water on the premises. There are no firearms or ammunition allowed or stored on the premises. Disinfectants, cleaning solutions, medication and other hazardous items are made inaccessible to the childrenin care. No poisons were observed during today’s inspection.

LPA Castellanos observed one fully charged fire extinguisher in the facility’s kitchen. LPA observed 3 working dual smoke and carbon monoxide detectors throughout the infant designation areas.

Furniture and equipment are in good condition, free of sharp, loose or pointed parts. Playground equipment is in safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. Floors in the infant rooms are clean and safe. Areas around high climbing equipment have cushioning material to absorb falls.

LPA Castellanos conducted a Health and Safety inspection of the kitchen, and all kitchen food preparation and storage areas are clean, free of litter/rubbish and free of rodents/vermin. All food is protected against contamination and any contaminated food is discarded immediately. Solid waste storage containers have tight-fitting covers and are in good repair. The facility is free of flies, insects, and rodents.

The facility does not provide meals or snacks, only filtered water. Per Site Supervisor parents assure they provide the meals and snacks for their infants, which comply with dietary restrictions and allergies.

Prior to working or volunteering in a licensed childcare facility, all individuals subject to a criminal record review have received a criminal record clearance or exemption. Upon notification from the Department, the licensee will comply and act immediately to terminate the employment of, remove from the facility or bar from entering the facility for any person it is deemed necessary while the Department considers granting or denying an exemption. Capacity and limitations as specified on the license are being maintained during today’s inspection. Facility was able to provide documentation that at least one person is trained in CPR and Pediatric First Aid and present when children are at the facility or at off-site activities.

All children are under supervision, including visual supervision, of a teacher at all times. There is a ratio of one teacher supervising no more than four infants in care. LPA reviewed a sample of children’s files and observed files were complete with contact information for authorized representative and or relatives or others who can assume responsibility for the child, medical assessment, individual feeding plan, and Infant Needs and Services Plan.

LPA reviewed a sample of staff files and observed files to be complete, records contain documentation of meeting qualification requirements.

The facility has sufficient age-appropriate furniture, and equipment including cribs, cots or mats, changing stations and a feeding table. There is indoor and outdoor activity space for infants that is physically separate. Each crib, mat or cot is occupied by only one infant at time and cribs are kept free from all loose articles including blankets and pillows and there are no objects hanging above or attached to the crib. Infants are not swaddled while in care.

Staff physically checks on sleeping infants every fifteen minutes and documents any signs of distress which includes but is not limited to flushed skin color, increase in body temperature, restlessness and labored breathing. Documentation for infants up to 12 months includes sleeping position if it is other than on their back. Individual Infant Sleeping Plan is completed and in file for each infant up to 12 months of age. Infants up to 12 months of age are placed on their backs for sleeping. Staff-infant ratio requirements are being met while infants are sleeping.

Per Director, Community Care Licensing was notified that the lead water testing conducted at the facility did not exceed the allowable limit for lead 5.5 ppb/ug, the samples tested met regulations. LPA Castellanos will further review and verify that the lead testing was completed in accordance with Written Directives outlined in PIN 21-21.1-CCP.

Site Supervisor was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Incidental Medical Services (IMS) is currently not being provided. Licensee is aware that an IMS plan is required to be submitted to the licensing office if they provide any of these services. Site Supervisor was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California.

LPA and the Site Supervisor discussed the Community Care Licensing website www.ccld.ca.gov which will provide access to Provider Information Notices (PINs), Quarterly Updates, COVID-19 Information and Resources, Mandated Reporter Training, Safe Sleep in Child Care, Lead Poisoning Facts, Forms and Regulations.
