Licensing Program Analyst (LPA) Shandra Powell conducted an unannounced Annual Random inspection. LPA met with Diane Avila, Director. Director was unable to guide analyst on a tour of the facility during inspection. Director was the Teacher in Pre-K room #2. This is a preschool program which consists of 2 classrooms; RM Pre-K 1 and RM Pre-K 2. This facility operates from 7:45 AM to 2:45 PM Monday thru Friday. The facility sits on the grounds of the Ascension Parish School.

All areas identified on the Facility Sketch were inspected. Upon arrival, the following staff were present during this inspection: RM Pre-K1: Staff #1, with 8 preschoolers; Room Pre-K 2: Staff #2, with 12 preschoolers. The following was observed during the tour of facility:

Furniture and equipment were inspected for age appropriateness and good repair. Telephone service, heating, lighting and ventilation were evaluated. Children have their own cubby to store their belongings. Children nap at this facility using cots. Per Director, the children take bedding home weekly for cleaning. The isolation area is located in the office. Age appropriate sinks and toilets were inspected for availability and good repair in all restrooms. General sanitation was observed. Availability of indoor drinking water was not available to children in classrooms.



LPA observed Disinfectants, cleaning solutions, paint and poisons (Home Defense) located in both classrooms under sink with unlock cabinet. This poses an immediately Health and Safety Risk to children in care. Per Director no medication is being administered at this time. LPA observed First Aid Kit. Tweezers were missing out of kit. All kitchen areas/food preparation areas and food storage areas are kept clean and are free of litter, rubbish, rodents, and/or any other vermin. All storage containers for solid waste, including moveable bins shall have tight-fitting covers that are kept on, and in good repair. Trash cans used to discard food have tight fitting lids.

LPA advised the licensee to access forms, regulations and quarterly updates on the Child Care Licensing website at: www.ccld.ca.gov. UPDATE: Parent’s Rights Poster (with complaint hotline). LPA advised licensee of the Mandated Reporter Child Care Training that became in effect January 1, 2018, and to maintain a copy of the certificate on file for each staff.

Licensee was made aware of The Child Care Advocate Program (CCAP) that is administered from within the Community Care Licensing Division. CCAP participates in many community activities and special projects in order to disseminate information on the State’s licensing role, provide information to the public and parents on child care licensing, and provide many other helpful resources to the licensees and the public. Mandated Reporter: Beginning on January 1, 2018, AB 1207, requires all licensed providers, applicants, directors and employees to complete training as specified on their mandated reporter duties and to renew their training every two years. Volunteers are encouraged but not required to take the training. Website: www.mandatedreporterca.com



Upon receipt of this report documenting a substantiated complaint allegation and a Type A deficiency, the Licensee shall do the following:

1. Post the Notice of Site visit and any licensing report documenting a Type “A” deficiency.
2. The report and the Notice of Site visit shall be posted for 30 consecutive days. Failure to maintain posting as required, will result in an immediate $100 civil penalty.
3. A copy of this report shall be provided to the parent/guardian of children currently enrolled by the next business day or immediately upon return. A copy of this report shall also be provided to the parent/guardian of any newly enrolled children for the next 12 months (1 year).
4. The Acknowledgement form (LIC 9224) must be maintained in each child’s file immediately upon receipt from parent. A copy of the parent Acknowledgement of Receipt of Licensing Reports Form was provided during this visit.

Outdoor playground equipment is in safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All areas around or under high climbing equipment, swings, slides, and similar equipment are cushioned with material that absorbs a fall. There is adequate shade in the play yard. Availability of outdoor drinking water was observed.

All individuals present have obtained a criminal record clearance or criminal record exemption.

LPA attempted to review Children’s Records for completeness; No children files were complete with required forms during this inspection this poses a potential health and safety risk to children in care.

Children's roster was not available for review. Sign-In and Sign-Out sheets were reviewed, LPA viewed one child not signed in that was present in Pre K Room #2 during inspection. This poses a potential health and safety to children in care. Disaster drill log was not available for review, per Director last drill was conducted on 05/09/19.

Menus are posted one week in advance where it is visible by the child's authorized representative. Menus for the past 30 days are available upon request. Morning snack and lunch are provided by facility. Evening snack is provided by child's Representative.

According to the Director, medication is only administered to a child when accompanied with a doctor's note and is stored in the upper cabinets of class room currently there are no children on medication.

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226.
