This report supersedes previous report*******
On 10/24/2019 12:30 pm, Licensing Program Analysts (LPAs) Denise Miranda and Lisa Rios arrived at Artadi Family Child Care Home located at 2542 S. Bundy Drive, Los Angeles, CA 90064 for the purpose of delivering the investigation findings. LPAs met with Eloise Artadi, licensee, and discussed the purpose of the visit. LPAs observed 8 children (which 3 were infants) in care being supervised by licensee and two (2) other adults. LPAs verified that all adults present in the home have obtained criminal record clearances and are associated to the facility.Based on LPA's observations at the facility, interviews conducted, review of documents obtained, and other evidences obtained throughout the course of the investigation, the allegations that Infant sustained multiple unexplained injuries while in care, Infant restrained in a high chair, Child's diapering needs not met resulting in a urinary tract infection. A finding that the complaints are unsubstantiated means that although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violations occurred. An exit interview was conducted and a copy of this report along with the Notice of Site Visit were provided to Eloise Artadi, licensee.
