Licensing Program Analysts (LPA) Alicia Mooberry conducted an unannounced annual required inspection at the above facility on 9/15/22 at 9:30AM. LPA met with Carla Elliot, Licensee, who guided analysts on a tour of the facility. There were 7 child and 2 adult present when LPA arrived. This facility operates using the Montessori model. Facility capacity is in compliance for a large Family Child Care Home. All adults present have obtained a criminal record clearance or exemption. Individuals residing in the home have been discussed and noted. LPA confirmed current telephone number and email address.

This is a one-story home which consists of three (3) bedrooms, one (1) bathroom, kitchen with laundry aea, dining room, living room, detached garage front yard and backyard (fenced). Main care is provided in the living room and dining room, one bedroom and backyard. The children use the bathroom located in bedroom 1 at front of home.

Per Licensee, areas off limits to children and parents include: Bedroom 2 and 3 by the kitchen entrance, front yard, detached garage.

Hours of operation are Mon-Fri 8am-5:00pm. Licensee provides food for the children. Licensee was reminded if children bring food from home it must be labeled with the child’s name and properly stored or refrigerated.

All areas identified on the facility sketch that are accessible for children to use were inspected for safety, comfort, and cleanliness. The following was observed and reviewed during this inspection:

LPA reviewed required posted documentation for Facility License, Publication (PUB) 394- Notification of Parent Rights and Licensing Form (LIC) 9148- Earthquake Preparedness form. Facility records were reviewed for LIC 9040- Facility Roster, LIC 610- Facility Disaster Plan and Disaster drill log.

Smoke and carbon monoxide detectors were tested and are operable. Fire extinguisher indicated fully charged but has not been serviced in the last 12 months. Licensee was reminded that fire extinguisher needs to be serviced yearly. The home is observed to be clean and orderly. There are toys and other age appropriate material available for children.

LPA observed that cleaning compounds are in kitchen cabinet under the sink with safety lock inaccessible to children. Licensee was reminded to ensure cleaning compounds and detergents are always maintained inaccessible to children in care, since the laundry area is located in the kitchen space. LPA observed cleaning products inaccessible to small children on overhead shelf in the bathroom that children use.

Licensee states that there are no poisons stored in the home and understands that all poisons must be lock, not only inaccessible to children.


Isolation area for sick children waiting to be picked up is in living room, away from the other children.

Licensee has no infants enrolled. LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Currently, children are using the back yard for outdoor play. The outdoor play area was observed to be fenced and composed of multiple sectioned areas for various uses. LPA observed that the outdoor yard has educational materials for children. LPA observed a small water feature that does not allow for pooled water. LPA did not observe any bodies of water, such as pools, spa or fountains. License was advised to always supervise children while playing with water. Per licensee, there are 2 cats in the home.

Children’s records were reviewed for (LIC) 282- Affidavit Regarding Liability Insurance, Immunization's Records, LIC 700- Identification and Emergency Information, LIC 627- Consent for Medical Treatment, LIC 995A Notification of Parents’ Rights, and documentation of 15-minute Infant Sleep Check (0-24 months)


Staff records were reviewed for approved Pediatric First Aid and CPR certification, LIC-501: Personnel. Child #4 is missing immunization records.

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Family Child Care Homes Section 102417. Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.
