25 | On 1/23/2020 at 9:05 AM Licensing Program Analyst (LPA) Angelica Ramirez conducted an unannounced inspection for the purpose of a case management-annual continuation. During the inspection conducted on 12/19/2019 LPA Ramirez was unable to review facility files due to the facility keeps their files off site. Therefore LPA will review files during today's inspection. Upon arrival, LPA met with Director Hamdam "Shanaz" Hoorfar and Office Manager Carol Daroy. LPA reminded Director and Office Manager that files must be made accessible within 30 minutes for review by licensing even if kept off site.
Director Shanaz guided the LPA on a tour of the facility. LPA observed six children (ages 2-3 years old) with two teachers in the Shooting Stars classroom, 16 children (ages 3-5 years old) with four teachers in the Super Nova classroom, and 12 children with one teacher in the Kinder-first grade classroom. All adults present today are cleared per the Licensing Information System (LIS).
There is an elementary school located on the same property, it is not licensed and does not fall under licensing regulations. Per Director Shanaz and Office Manager Carol, the kinder-first grade classroom falls under the elementary school. LPA discussed with Shanaz and Carol if the elementary school is part of a Private School Affidavit to which staff advised they will need to contact their superiors. LPA requested staff follow up with LPA Ramirez and provide a copy of the Private School Affidavit if it in fact is such.
