On 4/27/2023, Licensing Program Analyst (LPA) Suzette Ornelas conducted an unannounced visit for the purpose of delivering the findings for the above-mentioned allegations. LPA Ornelas met with Director, JERRILYNN FORD. LPA toured the facility and observed 11 children in care being supervised by 4 staff.

During the course of the investigation, LPA Ornelas made observations, reviewed parent handbook and conducted children, parent and teacher interviews in regard to the above allegations.

On 02/10/2023 LPA Ornelas conducted an unannounced visit at the Child Care Center (CCC). LPA
observed children in care engaged in different activities while interacting with staff and speaking freely to each other and teachers, being taken to the restroom as needed and being provided with comfortable accommodations. LPA did not observe any staff threatening day care children or demonstrating inappropriate forms of discipline.

Children in care are reporting that they like their teachers and that they are nice. Parents are reporting that
they have no concerns regarding the care and supervision that their children are receiving at the CCC.
Parents are reporting that their children are happy and like the CCC. Parents are reporting that Director and teachers communicate regular information regarding their child.

Based on the evidence obtained over the course of the investigation, LPA Ornelas was unable to corroborate that the Allegations, (1) Staff are denying the daycare children access to the restroom; (2) Staff are demonstrating inappropriate forms of discipline; (3) Staff are threatening the daycare children; (4) Staff are not providing the daycare children with comfortable accommodations while in care. Therefore, the allegations are determined Unsubstantiated. A finding that the complaint is unsubstantiated means that although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the allegation occurred.

An exit interview was conducted, and a copy of this report, appeal rights along with the Notice of Site Visit were provided to Director, Jerrilynn Ford.
