On 11/19/2021, Licensing Program Analysts (LPAs) Adrian Risher and Lillian Casillas, conducted a complaint visit regarding the above-mentioned allegation. Upon arrival, LPA Risher and Casillas met with Kye Lee Director. LPA explained the purpose of the inspection. LPA observed 9 kids in care with 2 staff.
On 11/19/2021, ESRO received a complaint regarding a child wandering away from daycare. RP stated that Child 1 was found outside by another facility that is about 2 blocks away.
Staff stated that 2 girls found Child 1 outside of the facility. Staff stated child 1 was in the bathroom and went out the door that leads to the nap room. Child 1 walked out the front door of the facility. Staff stated child 1 was found by 2 older girls walking down the street. Based on the investigation which included interviews with relevant parties and observations by the LPA, the preponderance of evidence standard has been met, therefore the above allegation is found to be Substantiated. California Code of Regulations Section 101229 is being cited on the attached LIC 9099D page.
Exit interview was conducted and a copy of the report was provided. Appeal rights were reviewed and provided. A plan of correction was discussed and provided to the Licensee.

Director will submit a declaration stating how you will keep the children safe while in care. LPA will provide video links via email for Care and Supervision Section 101229. Director will complete this by 12/01/2021 and provide verification via email.

This requirement was not met as evidenced by:
Child 1 was found wandering outside of the facility. Director, Administrator and Staff 1 admitted that Child 1 was found outside and walked down to another daycare facility.
