13 | On 04/18/2024 at 3:00 PM Licensing Program Analyst (LPA) Katrina Chicote conducted an Unannounced Complaint Inspection for the purpose of delivering findings to the above allegations. Census was taken.
During the course of the investigation, LPA made observations on multiple visits, conducted a variety of interviews of staff, children, and parents both currently and no longer attending, and obtained records. Multiple interviews provided corroborating information regards to above allegations with A4 being observed exhibiting these types of behaviors. Based on interview with Assistant Director, staff did disclose incidents involving A4 and LPA observed Assistant Director’s signed written statement in regards to disclosures made by staff.
Based on the available information, the preponderance of evidence standard has been met, therefore the above allegations are found to be substantiated. A substantiated finding means that the allegation is valid because the preponderance of the evidence standard has been met.
