Licensing Program Analyst (LPA) Isabel Ortega conducted an unannounced annual random site inspection. The LPA met with facility Education Program Manager, Rachel Chalmers who guided the analyst on a tour of the facility. Upon arrival LPA observed 68 children in care and 13 staff.
The operating hours of the center are Monday through Friday from 8:00 a.m. to 3:00 p.m. This is a State preschool center. The center offers full day educational services. The facility utilizes four classrooms for the care of children. This facility does not transport children.

The inside and outside of the center were inspected. Furniture and equipment were inspected for age appropriateness and good repair. Telephone service, heating, lighting and ventilation were evaluated. The LPA observed individual cubbies with children’s names. Children are provided with cots for nap time and LPA severed cots and blankets labeled and individually stored. The floors and area rugs are kept clean and safe. Disinfectants, cleaning solutions, and other items that pose a danger to children are inaccessible to children and stored in an off limits area and maintained locked with a child safety lock.

The center utilizes the restroom for children under care. Age appropriate sinks and toilets were inspected for availability and good repair. All toilets, hand washing facilities were in safe and sanitary operating condition. Toilet paper, foam soap dispenser, and paper towels were observed inside the restroom.

First Aid supplies were observed and complete. Smoke and carbon monoxide detectors(Dual) meet the fire marshal standards. Fire extinguishers were observed and in operable condition last serviced on 11/04/19. Fire and disaster drills are conducted on a monthly bases last emergency drill was recorded on 10/22/19 at 2:20 p.m. Children who become ill are isolated in a supervised office room away from the rest of the children and a toilet is designated for ill child/ren which is disinfected after every use.

The center provides breakfast, lunch and snack. A menu is posted inside each classroom. LPA observed a food allergy list posted in the kitchen area. Trash bins with tight fitting lids for solid waste were also observed. Drinking water fountains are available inside each classroom. The kitchen, food preparation and storage area is maintained clean and each drawers and cabinets have a child safety lock.

The outside yard was inspected. There are no bodies of water observed in the playground area nor on the premises. The outdoor playground was inspected and was observed to be free of hazards, loose and sharp parts. There is a shaded area for play. During outdoor play children are provided with daily filtered water kept in igloo containers. The playground was observed to be properly gated all around. Equipment was inspected for safety, cushioning material, good repair and age appropriateness.

Children files were complete with all required licensing forms. Staff files are located in the center main office located in North Hollywood and will be reviewed at a later time. The facility roster was up to date, facility uses child plus to maintain child enrollment records. The sign in and out sheet was observed inside each classroom. All staff have a criminal record clearance. The center was in compliance with staff-child ratios. The Center Director is currently certified in pediatric CPR and first aid which expires on 06/17/2021 Mandated Reporter Training was completed on 2/13/18. The facility had all the required licensing forms posted.

Staff are aware of the following Senate Bills:
*All Licensing reports are available for review on-line and public information. Summary: Assembly Bill 2621 added Section 1596.819 to the Health and Safety Code, to require the Department to post certain licensing information for CCCs on its public internet website.

§1596.7995 (a) (1) Commencing September 1, 2016, a person shall not be employed or volunteer at a day care center if he or she has not been immunized against influenza, pertussis, and measles. Each employee and volunteer shall receive an influenza vaccination between August 1 and December 1 of each year.

Effective July 1, 2019 new immunization records and requirements were discussed with facility.

Incidental Medical Services (IMS) policy was discussed. According to center a meeting is held with the child's family and a individual plan is made accordingly for each child with an IMS plan. Center staff demonstrated CCL's approval sheet for children with an IMS plan.

On or before March 30, 2018, a person who, on January 1, 2018, is a licensed child care provider, administrator, or employee of a licensed child day care facility shall complete the mandated reporter training provided pursuant to paragraphs (2) and (3) of subdivision (a), and shall complete renewal mandated reporter training every two years following the date on which he or she completed the initial mandated reporter training.

The facility was in compliance per Title 22 and Title V regulations, no deficiencies will be cited today 11/15/19. Exit Interview conducted, a copy of this report, appeal rights, notice of site visit was provided to Rachel Chalmers, facility Education Program Manager.
