Licensing Program Analysts (LPAs) Mireya Garcia and Jose Guzman conducted an unannounced annual random inspection to the above facility. LPAs met with Samantha Silva, Head Teacher, who guided analysts on a tour of the facility. Principal Holguin was not available during this inspection. However, later arrived during this visit. This is an infant/toddler program which is operated by LAUSD and operates under California Code of Regulations, Title 5. The program consists of 2 classrooms; Room One, which is an infant class and Room Two, which is a toddler class. This program is located on the campus of Ramona High School and runs from 7:00 AM - 3:00 PM. All areas were identified on the Facility Sketch were inspected. The following staff were observed during this visit: Room One (infants): Staff #1, #2 and #3 with 4 infants; Room Two (toddlers): Staff #4 with 4 toddlers. The following was observed during the tour of facility:

PHYSICAL PLANT

Disinfectants, cleaning solutions, medication and other items that are dangerous to children, were inaccessible to children. Head Teacher Samantha Silva states that there are no poisons stored at the facility. Furniture and equipment are in good condition, free of sharp, loose, or pointed parts. All toilets and hand washing facilities are safe, sanitary and are operating properly. All floors are clean and safe.

All kitchen areas/food preparation areas and food storage areas are kept clean and are free of litter, rubbish and rodents and/or any other vermin. All storage containers for solid waste, including moveable bins shall have tight-fitting covers that are kept on, and in good repair. Trash cans used to discard food have tight fitting lids. Drinking water is readily available both indoors and outdoors. The facility was observed to be free of flies, other insects and rodents.

Outdoor playground equipment is in safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All areas around or under high climbing equipment, swings, slides, and similar equipment are cushioned with material that absorbs a fall. There is adequate shade in the play yard.

INFANT PROGRAM
Infant changing tables have a padded surface no less than one inch thick and are covered with washable vinyl or plastic and have raised sides at least three inches high. Toys were observed to be clean and safe. Infant napping equipment that meets the requirements of 101439.1 (a) - (f). Indoor activity space for this infant program is physically separate from the other components at this facility, and meets the requirements of 101438.3 (a) - (e).

The facility is in compliance with the staff-infant ratios. A fully qualified teacher is directly supervising no more than 12 infants. Each aide is responsible for the direct care and supervision of a group of no more than four infants. Individual Feeding Plans are on file for each infant. Infant Needs and Services Plan are on file for each infant. Teachers present have proof of the required infant units on file and meet the qualification requirements.

The facility was observed to be in compliance with the staff-infant ratios required in 101416.5 (a) - (e). A fully qualified teacher is directly supervising no more than 12 infants. Infant Head Teacher present has a Child Development Permit on file, which includes the required infant units.

FACILITY RECORDS

All individuals present have obtained a criminal record clearance or criminal record exemption. All staff have received an active criminal record clearance as a condition of their employment with the Los Angeles Unified School District. There is at least one person trained in CPR and Pediatric First Aid present during this inspection.

In review of children’s records, LPA observed that child #1, #2, and #3 were missing LIC 613 A personal rights receipt on file. Files contain information including, but not limited to the following: Name, address and telephone number of the child's authorized representative and of relatives or others who can assume responsibility for the child if the authorized representative cannot be reached when necessary. LPA issued a copy of the LIC 857 Children’s Records Review and the LIC859 Staff Records Review to the Head Teacher & Principal during this inspection. REPORT CONTINUES ON THE NEXT PAGE 2 OF 3

Children's roster was reviewed and is current. Children present were signed in. Disaster drill log was available, last drill was conducted on 10/17/19.

Menus are posted one week in advance where it is visible by the child's authorized representative. Menus for the past 30 days are available upon request. Snacks were reviewed for availability, quantity and appropriateness to children in care. The facility provides children with breakfast, lunch and pm snack by the Los Angeles Unified School District.

The facility shall ensure that each infant is never left unattended, and is under the direct visual supervision of a staff person at all times;

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. The following information regarding ADA was provided: US Department of Justice (USDOJ) toll-free ADA Information Line at (800) 514-0301 (voice)/ (800) 514-0383 (TTY) and link to publication: Commonly Asked Questions about Child Care Centers and the ADA, available at: http://www.ada.gov/childqanda.htm.

Head Teacher states that there are no children currently enrolled that require these Incidental Medical Services.

LPA advised the Head Teacher & Principal to access forms, regulations and quarterly updates on the Child Care Licensing website at: www.ccld.ca.gov. UPDATE: Parent’s Rights Poster (with complaint hotline) was provided to the Head Teacher & Principal during this inspection. LPAs provided the following documents about SIDS: PIN 19-02-CCP (English and Spanish version) A Child Care Provider’s Guide to Safe Sleep from the American Academy of Pediatrics. LPA’s advised the Head Teacher & Principal to share PIN 19-02-CCP with Staff and Parent’s.

The following deficiency listed on the attached LIC 809D is being cited in accordance with California Code of Regulations Title 22.

The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. Exit interview was conducted with Samantha Silva, Head Teacher and Scarlett Holguin, Principal, including, but not limited to Provider Rights, Appeal Procedures and Agencies Consultative Role.
