On October 12, 2022 at 8:00 AM, Licensing Program Analyst (LPA) Monique Ayala conducted an unannounced inspection at the facility noted above and met with Director, Tracie Adams. The purpose of the inspection was to conduct the case management inspection for a pending Capacity Change application. This facility is currently licensed for 8 school-age children and is requesting to increase their capacity to 17 school-age children. This facility provides before and after school care, and consists of 1 classroom. The operating hours for this program is Monday through Friday from 6:00 AM to 8:00 AM and 1:00 PM to 6:00 PM. This facility also has a licensed infant center (License Number: 197412438) and licensed preschool center (License Number: 197412437) on-site.

All areas identified on the facility sketch were inspected. Upon arrival, no children or staff were present. LPA was guided on a tour of the facility accompanied by Director, Tracie Adams. The following was observed during the tour of the facility:

The following documents were posted in a prominent, publicly accessible area at the facility: Facility License, Notification of Parents' Rights (PUB 394), Earthquake Preparedness (LIC 9148), Child Passenger Restraint System Poster (PUB 269), Personal Rights (LIC 613A), Facility Sketch (LIC 999), Menu, and Daily Activity Schedule.

Furniture and equipment were inspected for age appropriateness and good repair. Telephone service, heating, lighting and ventilation were evaluated. Children have their own cubby to store their belongings. First Aid supplies were observed and kept in the classroom. Per Director, medication is only administered to a child when accompanied with a doctor's note and medication is stored in the medicine cabinet located in the classroom or in the kitchen.

Children's roster was reviewed and is current. Sign-in and sign-out sheets were reviewed. Per Director, staff sign the children in upon arrival to the facility after being picked up from school. Transportation is provided by the facility, and the facility has the proper seat restraints available for children. The facility has verification of fire and disaster drills available, and are conducted on a monthly basis. The facility provides children with snacks and meals. Food menus are posted one week in advance where it is visible by the child’s authorized representative. Menus for the past 30 days are available upon request. During the inspection, snacks were reviewed for availability and quantity.

Disinfectants, cleaning solutions, medication and other items that are dangerous to children, were inaccessible to children in all areas. Director stated that there are no poisons stored at the facility. The kitchen/food preparation area and food storage areas are kept clean and are free of litter, rubbish, rodents, and/or any other vermin. All storage containers for solid waste have tight-fitting covers that are kept on and in good repair.

Per Director, the isolation area is located in the office. In the office, a cot can be placed on the floor for an ill child and the staff restroom is readily available if an ill child requires it. Age appropriate sinks and toilets were inspected for availability and good repair. General sanitation was observed. Availability of indoor and outdoor drinking water was observed (water jugs).

The facility has a carbon monoxide detector that was tested and is operable. Smoke detectors were not tested during this visit due to the smoke detectors being wired to the emergency pull down fire alarm. The facility was granted a fire clearance for the requested capacity on September 15, 2022. The fire extinguishers are fully charged and were last serviced on February 15, 2022. The Director has a valid Pediatric First Aid/CPR certification (EMSA-approved) during this inspection.

Director was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. LPA advised the Director on how to access forms, regulations, quarterly updates, and Provider Information Notices (PINs) on the Department website at: www.ccld.ca.gov.

Based on measurements taken during today's inspection, including sink and toilet availability the facility is equipped for 15 school-age children inside the classroom and 4 school-age children at a time in the outdoor play yard. Per Director, a waiver request will be submitted to the Department with a staggered schedule to accommodate a total of 15 school-age children.

The Director will submit the following documents to the Monterey Park Child Care Regional Office by Tuesday, October 18th:
  1. Updated Application for a Child Care Center License (LIC 200A),
  2. Updated Designation of Facility Responsibility (LIC 308),
  3. Updated Personnel Report (LIC 500),
  4. Updated Emergency Disaster Plan (LIC 610), and
  5. Waiver Request for the Outdoor Play Yard with a Staggered Schedule.

A Notice of Site Visit (LIC 9213) was given and must remain posted for 30 days. No deficiencies were cited during today's inspection.
