On 5/12/2022,12:01pm, Licensing Program Analyst (LPA) Denise Miranda, conducted an unannounced Annual Required Inspection for the infant license. LPA met with Director, Brittney Adams, and toured the facility indoors and outdoors. Days and hours of operation are Monday-Thursday 7:00 AM – 4:30 PM and Friday 7:00AM – 3:30PM. LPA, observed 9 children in care and 2 teachers and Director providing supervision and care (3 infants with one staff and Director and 6 Toddler with one teacher. Facility checklist (LIC 125) provided upon arrival to Director.

There were 2 classrooms (#SB102 – D – Toddler room) and (#SB 102-B Infant room, 1 napping room (#SB 102-C) and a room for lactating mothers (#SB 100). Facility is located on the Brentwood School Campus. The center provides care to employees of the school only.

License to serve eight (8) infants ages: 0-18 months in the infant program and nine (9) toddlers age 18months – 3 years of age in the toddler option. The department has granted a shared outdoor activity space; waiver and license shall be posted in a pertinent area. LPA observed the waiver been posted by the license certificate.

There is no swimming pool or other bodies of water on the premises. There are no firearms or ammunition allowed or stored on the premises. Disinfectants, cleaning solutions, medication and other hazardous items are made inaccessible. All poisons are kept in a locked storage area. No poisons were observed during the inspection.

Furniture and equipment are in good condition, free of sharp, loose or pointed parts. Playground equipment is in safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of

hazards. Toilet and potty chair and hand washing facilities are in safe and sanitary operating condition. Floors in the facility are clean and safe. Per Director, parents provide the infants' breakfast, lunch and snacks. LPA observed, sinks, refrigerator and microwave in both classrooms. This area are clean, free of litter/rubbish and free of rodents/vermin. All food is protected against contamination and any contaminated food is discarded immediately. Solid waste storage containers have tight-fitting covers and are in good repair. Drinking water is available both indoors and outdoors. Areas around high climbing equipment, slides have cushioning material to absorb falls. The facility is free of flies, insects and rodents. Facility has one or more functioning carbon monoxide detectors that meet statutory requirements. Prior to working or volunteering in a licensed child care facility, all individuals subject to a criminal record review have received a criminal record clearance or exemption.

Capacity and limitations as specified on the license are being maintained. At least one person trained in CPR and Pediatric First Aid is present when children are at the facility or at offsite activities. The name of the infant child care center director or fully-qualified teacher(s) designated to act in the director’s absence has been reported to the Department. The person who signs the child in/out of the facility shall use their full legal signature and record the time of day. All children are under supervision, including visual supervision, of a teacher at all times. Facility maintains a ratio of one teacher supervising no more than 4 infants and more than 6 toddler in care. LPA reviewed a sample of 9 children’s files and observed files were complete with contact information for authorized representative and or relatives or others who can assume responsibility for the child and medical assessment. LPA reviewed all staff files and observed files were complete with health screening, immunization records for influenza, pertussis and measles and current documentation of completed mandated reporter training. Menus are posted at least one week in advance where an authorized representative can view them.

Criminal Record Statement :Director was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Incidental Medical Services (IMS) policy was discussed.
Regulation and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. LPA and Director discussed the Community Care Licensing website www.ccld.ca.gov which will provide access to Provider Information Notices (PINs), Quarterly Updates, COVID-19 Information and Resources, Mandated Reporter Training, Safe Sleep in Child Care, Lead Poisoning Facts, Forms and Regulations.

Staff interview conducted and documented at 2:03pm.

Per Title 22, Division 12, Chapter 3, of the California Code of Regulations, no deficiencies are cited.

This report shall be made available to the public upon request. LIC 9213 Notice of Site Visit is provided and required to be posted for 30 days.

Exit interview conducted and report was reviewed with the Director. Copy of the report, Appeal Rights and Notice of Site Visit were given to Brittney Adams, Director.
