Licensing Program Analyst (LPA) Alicia Mooberry conducted an unannounced Required - 3 Year inspection on this date. LPA met with Luana Madrigal, Director, LPA explained the purpose of the inspection and was guided on tour of the facility. This is an Infant program serving up to 15 infants located in Room #1 of facility that has Preschool license #197417433. Indoor and outdoor infant spaces are physically separate from other child care components. The entrance to the facility is locked and parents enter using a doorbell. There were 10 infants in care during visit supervised by 2 teachers. Teacher-child ratios are in compliance and staff fingerprints have the required clearances. The facility operates Monday – Friday from 8:30 AM – 5:00 PM and offers a half day schedule 9:00am to 12:00pm.

LPA observed required licensing documents at entrance to the facility. Furniture and equipment were inspected for age appropriateness and good repair. Toys were observed to be safe, and do not have sharp points, edges, splinters and are not made of small parts that can pose a choking hazard. LPA did not observe any baby walkers or any items that fall into that category present in the facility. The classroom was inspected to ensure that the floors have a surface that is safe and clean. LPA observed children have their own cubby to store their belongings in each classroom. Napping area was observed to be physically separate from the indoor activity space. The facility was observed to be maintained at comfortable temperature.

LPA observed one crib per child available. Per Director, at least one teacher is always supervising the nap room while infants sleep. Napping mat/cot are available for infants who are able to climb out of the crib.



The diaper changing table in of the classroom has raised sides that are at least 3 inches high with surface that is easy to wipe. LPA advised Director of regulation stating sink should be within arms reach. General sanitation was observed. Cleaning supplies are kept locked and/or inaccessible.

Outdoor play area is fenced, physically separate from the preschool and free of hazards including holes, sharp objects, debris, animal droppings and dry grasses that pose a fire hazard. All play equipment, toys, materials and surfaces accessible to children appear to be age appropriate and toxic free. There is adequate shade on the majority of the play area. Per Director, the equipment is checked to ensure it is not too hot for children in care. LPA discussed never leaving children unattended at any time. Extra precautions if facility does water play.

Snack menus are posted at least one week in advance and are visible to an authorized representative. The facility provides AM and PM snacks and lunch is provided by parents and/or outside vendor. Snacks were reviewed for availability, quantity, expiration dates and appropriateness for children in care. LPA observed the snack storage area (kitchen) appear to be in sanitary condition. LPA observed trash receptacles have tight fitting covers.

The facility has one or more functioning smoke detectors and carbon monoxide detectors that meet statutory requirements. LPA observed the required 2A10BC fire extinguisher serviced within the last 12months..

LPA reviewed sign in and sign out sheets to ensure children are being properly signed in and signed out by the authorized representative. LPA conducted a Children's Records Review for completeness and to ensure children have proper forms such as Identification and Emergency forms, Immunization records, Health History, Personal Rights, Parents Rights, Sleep Plan (LIC 9227) and sleep plan on file. The children's roster was available and current.

Staff Records were reviewed to ensure the following: Criminal record clearance or criminal record exemption, education credits required for Infant Care, current CPR/First Aid and health preventative practices, proof of immunization against pertussis and measles and influenza and/or flu declination, Mandated Reporter training. LPA observed at least one staff member to have current EMSA approved pediatric CPR/1st Aid.

LPA discussed the safe sleep regulations with facility representative and discussed the Child Care Licensing Safe Sleep webpage at: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep, as an additional resource. LPA also informed facility representative of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/, and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. ----- Page 2 Report Continues

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretation and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. LPA informed Director to log onto web site www.ccld.ca.gov to access forms, regulations and quarterly updates. Records for all children and staff must be maintained for three (3) years after separation from the facility. Report Unusual Incidents and/or injuries to the parent/guardian immediately and to Child Care Licensing within 24 hours and/or the next business day.

The Facility Representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

LPA verified that the lead testing was completed and in accordance to the Written Directives outlined in PIN 21-21.1-CCP.

There were no violations of Title 22 regulations at this time.

Facility representative was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

A notice of site visit was given and must remain posted for 30 days.
