Licensing Program Analyst (LPA) Maddox conducted an unannounced Annual/Random inspection. LPA met with Rachel Chalmers, Educ. Pgm Mgr and Edie Smith, Educ Area Supervisor, together we toured 4 classrooms which includes 2 additional classrooms center wishes to use or Early Head Start (for children ages 24 mos to 36 mos), 2 bathrooms, Kitchen, and outside play yard. During the time of this visit there were 28 children present ages 3 -4 yrs old in classrooms 116(01) and 113(02). The center is open from 7:45 am to 4:15 pm (with staggered operational hours for the EHS and HS classrooms.

Indoor Space: All disinfectants, cleaning solutions and other items that are dangerous to children were found to be inaccessible to children. All cleaning and disinfectants in the classrooms are stored underneath the sink with key lock and child proof locks.

Furniture and equipment were inspected for age appropriateness and are in good repair. Rooms inspected and are clean and safe. Telephone service was verified and operational. Heating, lighting, and ventilation are adequate. There are cubbies for children's belongings inside the classroom. LPA observed age appropriate toys and materials. Drinking water is readily available to children via water dispenser.

Outdoor play equipment was inspected for health and safety. LPA observed on the playground that there are climbing structures, slides and other large play equipment. All play equipment was found to be securely anchored, and there is adequate shade for the children. LPA did not observed any bodies of water on the premises. LPA observed foam rubber mat material underneath the play equipment. The surface of the outdoor activity space is in safe condition and free of hazards. Drinking water is readily available on the play yard LPA observed water fountains. The entire play yard is enclosed with fencing all around the perimeter, fencing exceeds required height of 4 feet high. Playground is free from miscellaneous debris such as tree branches, trash, leaves, etc.

The outside play yard was measured today so see how many children can occupy the yard at one time (The swing area on the play yard is off limit at this time and was no included in the outdoor measurement):
98 X 41 = 4,018/75 = 54 (an outside play schedule is in place)

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. Children are inspected for illnesses as they arrive including taking temperatures. Medications are kept in a safe condition, and free of hazards. A review of medication policy indicates that prescription and non-prescription medication is administered only with parent's written permission.

**Sign in and out sheets were reviewed -manually. The parent board was reviewed and has all of the required forms posted. Fire/earthquake drills current. LPA reviewed Children's records as part of this inspection, all staff files are kept at a central location (Maud Booth). LPA verified at least one staff person present has current Pediatric CPR and First Aid training (exp 6/26/23).



There is 1 bathroom between HS classes 01, and 02 (this bathroom has 3 toilets and 3 sinks), there is a second bathroom between Rooms EHS 01 and 02 (this bathroom has 3 toilets and 3 sinks however 1 toilet is not in use due to changing table in place). There are toilets and sinks that are functioning properly and are age appropriate. All toilets and hand washing facilities are in safe and sanitary condition. LPA observed soap, toilet paper and paper towels readily available, hot water is the appropriate temperature.

Food Service: Food is brought in daily by CACFP (Food Program). There is a clean, fully equipped kitchen with a refrigerator, freezer, microwave and stove. Menus are posted in each classroom and the kitchen (an allergy list is posted in each classroom and in the kitchen).

All chemicals in the kitchen were kept separate from the food. All cleaning solutions/chemicals and compounds in the kitchen were locked by key and also had child proof latches. Food that was stored in containers was labeled properly. The facility has ensured and establish measures to keep the facility free of flies and, insects and rodents.

LPA observed that this facility has an operating telephone. LPA observed that each classroom had a fully charged and operational 2:10ABC fire extinguisher and functioning carbon monoxide detectors that meets statutory requirements. All storage containers for solid waste, including moveable bins, have tight fitted covers that are kept on, and are in good repair.

This facility is in compliance with Title 22 Regulations no citations were cited at during this visit. A notice of site visit was left at the facility.
