Licensing Program Analyst (LPA) Tatiana Bickham conducted an unannounced 3 year required inspection to the above facility on 06/05/2025. LPA arrived at the facility at 2:00 PM, identified self and met with Arash Lorestani, Director, who guided LPA on a tour of the facility.

This is a school age program located at Woodlands Elementary Charter For Enriched Studies. LPA observed Woody's Clubhouse to have their own assigned bungalows with its own entrance located near the elementary outdoor yard. Per Director, days and hours of operation are Monday, Wednesday, Thursday, and Friday from 2:30pm to 6:00pm; Tuesdays from 1:30pm to 6:00pm.

All areas identified on the Facility Sketch were inspected. LPA toured the facility indoors and outdoors and a census was taken. The facility is currently using rooms 11, 12, 15, and 16.

There were 95 children under care and supervision of the Director and 9 other staff members. The facility was observed to be within the license capacity and limitations.

Furniture and equipment are in good condition, free of sharp, loose or pointed parts. Tables and chairs meet the needs of the children in care. LPA observed did not observe a carbon monoxide detector located in the classroom. Per Director the school has a combined smoke and carbon monoxide detector, LPA was unable to test the alarm.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

There are two outdoor areas, outdoor areas were observed to be located outside the bungalows. Outdoor area was observed to be free of any hazards.

LPA observed the facility to be free of flies, insects and rodents. No poisons were observed in today’s inspection. Disinfectants, cleaning solutions and other hazardous items were observed to be inaccessible to children in care.

All toilets and handwashing facilities are in safe and sanitary operating condition. Facility has separate restrooms for both sexes. Toilets used by school-age children provide individual privacy. Floors in the facility are clean and safe.

All kitchen areas/food preparation areas and food storage areas are kept clean and are free of litter, rubbish, rodents, and/or any other vermin. All storage containers for solid waste, including moveable bins have tight-fitting covers that are kept on, and in good repair. Trash cans used to discard food have tight fitting lids or solid waste bags shall be discarded immediately after each snack. The facility only provides a snack. First Aid supplies were observed. Per Director, medication is not administered at the facility.

Drinking water is available both indoors and outdoors with use of a water dispenser. Children have water bottles, and the facility has disposable cups available.



Facility has not conducted water lead testing as the children do not drink the water and they do not prepare food using schools water system.

LPA observed the following postings on the parent board located on the wall of the classroom: Facility License, LIC 613A (9/96), the PUB 393 (8/02), Child Care Center Notification of Parents' Rights Poster, LIC 610 Emergency Disaster Plan with the telephone number of the local health department, information on child passenger restraint systems. Menu was observed on the parent board.

LPA reviewed a sample of children’s files and observed files to be complete. Children’s records were reviewed for Admission Agreement, LIC 700- Identification and Emergency Information, LIC 627- Consent for Medical Treatment, LIC 995A Notification of Parents’ Rights, LIC 613-Personal Rights, LIC 702-Health

History, and LIC 701 Physicians Report. The person who signs the child in/out of the facility shall use their full legal signature and record the time of day. LPA observed sign out sheets with a section for parent/ guardian to sign out using a full legal signature.

LPA reviewed staff files show staff files to be complete with mandated reporter, immunization verification of Tdap, MMR, and flu, and health screening with TB clearance. Review of facility file shows Director has CPR/First Aid that expires 08/2025, LPA did not observe a Health and Safety Certificate for the Director. LPA observed during file review at 4:15pm one staff was not fingerprint cleared.

Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02- CCP. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Director was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Director was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

Based on the LPA’s observations and records review, the following deficiencies listed on the attached LIC 809D (deficiency page) are being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children’s health & safety. A violation regarding an uncleared adult warrants an immediate civil penalty of 500.00 and is hereby assessed, see LIC
