25 | On 5/26/2022 at 11:37 am Licensing Program Analyst (LPA) Deborah Lowe, conducted an unannounced Annual Required Inspection for the preschool license. Facility is located on a Los Angeles Unified School District (LAUSD) campus. LPA met with Sonia Gonzalez, School Administrative Assistant, LPA identified reason for visit. At 11:47 am LPA Lowe arrived at preschool classroom located in room #5, met with classroom S#1 and toured the facility indoors and outdoors. Days and hours of operation are Monday – Friday 8:00 am – 11:00 am and 11:30 pm – 2:30 pm.
LPA observed 4 children are under supervision, including visual supervision, of 3 adult staff.
There is no swimming pool or other bodies of water on the premises. Disinfectants, cleaning solutions, medication and other hazardous items are made inaccessible. No poisons were observed during the inspection.
Furniture and equipment are in good condition, free of sharp, loose or pointed parts. Playground equipment is in safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All toilets and handwashing facilities are in safe and sanitary operating condition. Floors in the facility are clean and safe.
Elementary school has a cafeteria that handles all food preparation. Solid waste storage containers have tight-fitting covers and are in good repair. Areas around high climbing equipment and slides have cushioning material to absorb falls. The facility is free of flies, insects and rodents.
Facility was observed with a fire life safety system with smoke detector and a second device on ceiling. Second device observed was not labeled and facility was unable to verify with written documentation the device as a carbon monoxide detector.
At 1:50 pm LPA observed facility is in need of one or more functioning carbon monoxide detectors that meet
