On 04/24/2024 at 1:11 p.m., Licensing Program Analyst (LPA) Doris Whitmore made an unannounced visit for the purpose of conducting a required 3-year inspection. During the inspection, LPA met with Pam Youngblood, Center Director from another facility, and advised them of the purpose of the inspection. A tour of the facility was provided inside and out. There were no children present at the time of the inspection. Today was a student free day. There was a total of 1 staff at the time of the visit. All center staff that was present during today’s visit has criminal record clearances and are associated to the designated license number. Facility has one classroom, Hours of Operation are Monday- Friday 7:30a.m. 3:30 p.m. LPA observed all postings requirements for operation on the postings board LIC203A- License, LIC610A-Emergency Disaster Plan, LIC9148- Earthquake Preparedness Checklist, PUB394-Notification of Parents Rights Poster, LIC613A- Personal Rights, PUB-269- Car Seat Law, Menus, Activity Schedule. LPA Whitmore checked the Personnel Report to make sure it was up to date.

LPA observed operable carbon monoxide, which were tested at the time of the inspection. Smoke Detector was not tested to high up in the ceiling. Also the smoke detector is connected to the fire alarm. Fully charged fire extinguishers, and fully stocked First Aid Kits were observed. LPA observed emergency back packs in the classrooms. Furniture and equipment were inspected for age appropriateness and good repair free of sharp, loose, or pointed parts.Telephone service, heating, lighting, and ventilation were evaluated. Napping equipment was observed to be clean and in good condition. Storage for children’s belongings were neat and organized. LPA observed toilets and sinks to accommodate the facility’s capacity. Floors are cleaned in the restroom. Sufficient supply of toilet paper, soap, paper towels, was observed. All trash cans had tight fitting lids. The facility is free of flies, insects, and rodents. LAUSD contracts with a pest control vendor to ensure that the facility stays pest free. The facility roster was up to date. The isolation area was inspected and located in the office. Drinking water is readily available and trash cans with tight fitting lids were outside observed in the classrooms.Sign in/out were reviewed and facility roster.Documentation of current Fire & Earthquake drill up-to-date. the emergency disaster plan is updated and reflects the current

operation. The outdoor area was inspected Playground equipment is in safe condition, free of sharp, loose, or pointed parts. LPA did not observe any debris or hazards that would pose an immediate risk to children in care.

LPA observed an age-appropriate climbing apparatus with proper cushioning beneath and properly secured. LPA checked playground equipment for ages. LPA observed shade in the outdoor play area. Drinking water is made available during outdoor play. There is no swimming pool or bodies of water on the premises. None were observed during the inspection. There are no firearms or ammunition allowed or stored on the premises. All electrical outlets were covered. LPA reviewed a sample of children’s files and observed 10 files completed. LPA reviewed all staff files. At least one person trained in CPR and Pediatric First Aid is present at the facility. LPA discussed the importance of making sure all Pediatric CPR and First Aid is EMSA approved. LPA Whitmore discussed about LIC311 A, LIC125, and Entrance Checklist to use as a reference when auditing files. Headstart is open to enroll children that might need medication administered. Headstart classrooms currently has children with allergies and/or medication.

LPA observed allergy chart posted in each classroom and kitchen.Breakfast /Snack/lunch menus were posted. Breakfast was delivered the day before and lunch and snacks were delivered daily from the main kitchen located Van Nuys. Food and snack items are properly stored. The food preparation area was toured. Meals are delivered daily from the main kitchen. The kitchen area which is made inaccessible to children in care. LPA observed the kitchen to be clean and equipped with a sufficient supply for meals. There were no cleaning compound compounds stored with the utensils. The facility serves breakfast, lunch, and snack. Proper food storage was observed. Food and snack items are properly stored. LPA Whitmore checked to make sure staff and children food is not in the same refrigerator. LPA reminded facility to ensure that all medication is stored with all necessary LIC forms All prescription and nonprescription medication shall be centrally stored in accordance with the following requirements medication shall be kept in a safe place inaccessible to the children in care. Safety Code section 1596.871, must obtain a criminal record clearance.or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For childcare center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP). LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP.



LPA discussed the safe sleep regulations with Pam Young Blood and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and- resources/safe-sleep as an additional resource. LPA also informed Pam Young Blood of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

This facility provides Incidental Medical Services- IMS. LPA reviewed storage of medication and equipment/ supplies, and reviewed children’s personnel and administrative records. Center Director Pam Young Blood was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain childcare by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.
