25 | Licensing Program Analyst (LPA) Rita Ramos conducted an unannounced case management inspection due to incidents that were reported to the department. LPA met with Pamela Torres, Site supervisor, who guided LPA on a tour of the facility. There were 15 children and 5 staff present upon arrival.
The purpose of the visit was to follow-up on incidents that was reported to the department. LPA conducted interviews, took pictures, and obtained documentation during this visit.
The incident that occurred on 03/08/22, was reported to the Department on 03/08/22, via telephone. The facility reported the Unusual Incident to the Department within the required 24 hours of occurrence. The incident that occurred on 01/19/22 was reported to the Department on 01/19/22, via telephone. The facility reported the Unusual Incident to the Department within the required 24 hours of occurrence.
The incident reported on 03/08/22 pertains to missing outdoor equipment and evidence of someone trespassing when children were not in care. The incident reported on 01/19/22 pertains to a parent encountering a transient after picking up their child from the facility. Facility staff contacted the local authorities.
Based upon information received during the inspection, it was determined that there is no violation of Title 22 to children in care. There were no deficiencies cited during today’s inspection.
The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
