On Wednesday, August 31,2022 at 12:23 p.m. Licensing Program Analysts Mayra Rivera and Patricia Medel conducted an annual inspection and met with Site Supervisor Candace Barber who guided LPA's Rivera and Medel on a tour of the facility.

This is a preschool program which consists of one classroom and outdoor play yard. This preschool program operates from Monday to Friday from 8:00 a.m. to 4:30 p.m. All areas identified on the facility sketch were toured and inspected both indoors and outdoors.



At approximately 12:35 p.m. LPA entered classroom 1 and observed a total of 12 preschool children napping. LPA Rivera observed 3 staff members present. LPA Rivera observed age appropriate furniture and equipment to be in good condition, free of sharp, not loose, or pointed parts and age appropriate toys accessible to children. LPA observed individual personal bin containers with children names inside the cubbies. LPA observed cleaning solutions stored inside the wall cabinet with a child proof lock in place making it inaccessible for children to open the cabinet doors. LPA Rivera observed three step trash bins with a tight-fitting lid.

At approximately 12:43 p.m LPA Rivera observed the bathroom located inside the classroom to be in good condition with working toilets. Outside the bathroom LPA observed 2 hand sinks, hand soap and paper dispenser. Ill/isolation area children utilize staff restroom located inside the classroom by the main entrance.

At approximately 1:02 p.m. LPA Rivera entered the outdoor playground to conduct an inspection. LPA observed the equipment to be age appropriate and in safe condition, free of sharp, no loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All areas around or under climbing equipment, are cushioned with granulated rubber cushion padding that absorbs a fall. For outdoor drinking water, the water jugs are available along with disposable water cups. For adequate shade, LPA observed a sun sail triangle. LPA Rivera observed a step trash bin with a tight-fitting lid.

LPA also absorbed three storage sheds. Two sheds were locked with a keypad lock and the other storage is utilized to store playground equipment. LPA observed two side gates closed with self-latch locks.

At approximately 1:25 p.m., heating lighting and ventilation were evaluated by LPA Rivera. LPA observed central AC/heater and vents located on the ceiling wall. LPA observed the thermostat located in the classroom and the current room temperature to be at 72 degrees. LPA observed the fire extinguishers located inside the classrooms and kitchen and the valve on the required 2A10BC fire extinguishers indicating fully charged and serviced on 2/24/2022. LPA Rivera did not test the pull-down alarm station. LPA did not want to activate the alarm therefore the alarms were not tested.

LPA did observed napping equipment during nap time. For ill/isolation area children utilize the director's office and a cot is available for children to rest if needed. At approximately 1:40 p.m. LPA Rivera reviewed staff, children files, children roster and sign in/sign out sheets. Children files are up to date and complete.

This program provides breakfast, lunch and p.m. snack. The meals are provided by outside vendor. A monthly snack menu is available and posted in the classroom. For water drinking LPA observed disposable water cups and a water dispenser inside the classroom. LPA observed the food preparation area to be clean and free of litter, rubbish, rodents, and/or any other vermin and storage containers for solid waste, including moveable bins with tight-fitting covers that are kept on, and in good repair. The facility was observed to be free of flies, other insects and rodents. No children with food allergies during this inspection.



Facility administers medication. Before administrating medication, parents sign a medication consent form, provides the medication in original container with Dr's prescription. At approximately 2:35 p.m LPA Rivera observed the medication stored inside the emergency with expiration date 1/26/23.

Incidental Medical Services (IMS)

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. Staff # 2 Pediatric First Aid /CPR certification expires on 1/09/2023.

Leas Teacher has child abused mandated certification (AB 1207) dated 8/22/22 and immunization against pertussis, and measles, and influenza declination. Lead Teacher has been informed mandated reporter training must be completed every 2 years, and is available at www.mandatedreporterca.com


Criminal Record Statement
Lead Teacher Trishona Smith was informed that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPA Rivera provided technical assistance for staff # 1 having the incorrect Pediatric First Aid/CPR certification and staff # 2 not having the required documents on site such LIC 503 Health Screening Report, LIC 501 Personnel Record, TB, MMR, TDAP, Influenza or Influenza declination, LIC 508 Criminal Record Statement. Staff 2 Pediatric First Aid/CPR certification
