On 3/17/2022 at 9:24AM, Licensing Program Analyst (LPA) Meghan McGee conducted an unannounced Case Management – COVID-19 inspection for the purpose of following up on 9 Unusual Incident Reports (UIR) submitted to the department between the following dates: 1/5/2022-1/18/2022. LPA McGee met with Health and Safety Director and Director, Nelly Rios and Nara Keheyan , who toured LPA through the inside and outside of the facility. LPA observed 25 children in care with 12 staff. Staff present have criminal record clearances on file.

According to the UIRs submitted from 1/5/2022-1/18/2022, 3 children and 7 staff members were reported as testing positive for COVID-19. Director stated that all children and 4 staff who were reported positive COVID-19 test results have all isolated and have since all returned to the facility. 2 staff were substitutes and are not present at the facility today and 1 permanent staff is out today.

LPA McGee observed COVID-19 signage throughout the facility for mask wearing, hand washing, and stay safe 6 feet apart. LPA observed parents enter through one main entrance. Parents complete health screening for child on app. LPA observed front desk staff check to make sure child health screening is complete. If child screening is not complete, front desk staff will ask health screening questions to clear child to enter facility for the day. Director stated the currently no outside visitors are allowed.

LPA observed facility supply of PPE COVID supplies: gloves, disinfectant wipes, adult and child mask, paper towels, bleach, and hand sanitizer. LPA McGee discussed with Director current COVID-19 resources and guidance. LPA emailed link for updated California Department of Public Health Guidance. LPA made aware to follow the local public health requirements if they require a higher level of masking.

No deficiencies were cited, per Title 22, Division 12, Chapter 3, of the California Code of Regulations.

An exit interview was conducted and a copy of this report and Notice of Site Visit were provided to Director, Nara Keheyan.
