13 | Licensing Program Analyst (LPA) Ortega conducted an subsequent inspection for the purpose of delivering the findings related to the complaint investigation allegation above. LPA met with Director, Sonia Sandoval and informed the facility of the purpose for the inspection. A census of the children was taken, and a tour of the facility was conducted. The complaint investigation consisted of interviews with the licensee, children, parents and recording footage and pictures.
The investigation revealed that during day care child#1 tripped(incorrect shoe size), bumped her head(running) and fell multiple times since enrollment at the facility. Foster parent disclosed she was notified every time child was injured in writing or verbally. Observation of the recording submitted revealed staff standing in close proximity to child and attending to the child. Although, child#1 was injured by tripping or accidentally bumping her head, supervision was provided and injuries were accidental. The interviews revealed that injuries were not intentional nor lack of supervision that could corroborate that the facility failed to provide supervision that resulted in multiple child injured while in care.
