On 2/23/2024 LPA Ortega conducted an unannounced Case Management Incident inspection and met with facility Director Sonia Sandoval. LPA disclosed the purpose of the inspection and was granted entry into the facility. During the time of inspection 62 children were present and 8 staff providing care and supervision.

The purpose of the inspection was to follow up on a self reported unusual incident report (UIR) which occurred on 2/20/2024 at the facility. The Department was notified of the UIR on 2/20/2024. According to the UIR, child #2 scratched child's #1 left cheek causing skin layer to peel. According to the UIR, nine children were present and Staff #1 was in the classroom.

During the inspection today, LPA interviewed staff members and children, and completed a health and safety check of the facility. Facility demonstrated the video footage dated 2/20/24 and submitted documentation related to the incident.

Children were observed watching a movie and sitting on the carpet next to staff #1 nine children were present and one staff. Child #1 was observed rubbing his side left cheek and then stood up and walked over to staff #1. Due to the camera and children's position child#1 and child #2 were not capture prior to occurrence leading to the morning incident of 2/20/2024. According to camera footage, photos and interviews conducted with children and staff, children were watching a dinosaur movie and staff #1 was sitting in a blue teacher chair providing supervision. Child #1 sustained an loss of epidermis skin on left cheek caused by child #2. Child #1 and Chid #2 were observed discussing the dinosaur movie. According to interviews child #2 had long uneven trimmed finger nails which caused child #2 to injure child #1.


LPA completed a safety inspection of the 5 classrooms. Zero deficiencies were observed.

During today's inspection, LPA obtained documentation related to the UIR. At this time, no further investigation is required.

An exit interview was conducted, a copy of this report, and notice of site visit were provided to the facility Director.
