Licensing Program Analyst (LPA) Silva Garibyan conducted an unannounced Annual/Required inspection. LPA met with Ziporah Leah Truly, Director who guided analyst on a tour of the facility. The facility operates a Pre-school (25) with Toddler Option (14) program. The facility operates Monday through Friday, from 8:30 AM to 2:30 PM with extended care until 5:00 PM. A review of the sign in/out sheet was conducted to verify the current census of children. Facility is operating within capacity limitations. The facility consists of 5 classrooms ( three preschool and two toddler option) , Director's office, Play yard, Courtyard, small shaded area between courtyard and play yard, and one children's bathroom. There is a separate kitchen for Preschool use. Director's office and staff bathroom are used as isolation areas for sick children. There are no bodies of water on the premises.

LPA observed the following classroom ratios: Circle Time Room (one Teacher and 8 preschool age children); Art Room (one Teacher, one Aide, and six toddlers). LPA observed teacher child ratios in compliance and staff have the required background clearances at this time.

LPA observed the following postings in Director's office: License, Playground waiver, Preschool and Toddler Daily Schedule, LIC 613A, the PUB 393, Child Care Center Notification of Parents' Rights Poster, LIC 610 Emergency Disaster Plan with the telephone number of the local health department, information on child passenger restraint systems (PUB269). Menus were observed to be in writing and posted. Furniture and equipment were inspected for age appropriateness and good repair. The classrooms were inspected to ensure that the floors have a surface that is safe and clean

LPA observed telephone service, heating, lighting and ventilation to be satisfactory. Children have adequate cubby storage for their belongings in each classroom. The parents provide sheets and blankets and laundered weekly or more often if needed. Age appropriate restroom, sinks(4) and toilets(4) were inspected for good repair, water temperature, toilet paper, safety and sanitation. All available toilets and sinks are in operable condition.

Availability of drinking water was observed via sippy cups. Sippy cups are taken outside to the play area for drinking. There are first aid supplies in each classroom and Director's office.

Children are inspected for illnesses as they arrive. A review of medication policy indicated that no medication is administered. Parents must administer the medication themselves or send an authorized person on their behalf. No medication is stored at the facility. There is a separate area for isolation and care of ill children in the director's office. There is a mat available for each ill child. An isolated toilet and sink are available for ill children.



There is a clean, fully equipped kitchen with stove, refrigerator/freezer, and microwave oven. The facility provides breakfast, lunch and snack. Menus and allergy lists are posted in the kitchen and classrooms. An appropriate amount of food and snacks were observed. Director purchases the food and milk weekly and meals are prepared on-site. There is hot and cold running water in the kitchen/food preparation area. Refrigerator is clean and operating at the proper temperatures. The chemicals are kept separate from the food. The kitchen and staff bathroom are inaccessible by means of a door knob covers. Solid waste storage containers have tight-fitting covers and are in good repair.

The play yard was observed to be free of debris. There are areas for shade and rest on the play yard. Two chickens and one rabbit was observed in the garden area. Drinking water is available. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. Per Director the opening teacher inspects the sandbox daily to keep it free of hazardous foreign materials. There is wall around the playground. LPA observed a gate(second exit) that leads to the alley.

The facility is equipped with a working smoke detectors and Carbon Monoxide detectors in all classrooms. LPA observed two fire extinguishers (2A 10BC) located throughout the facility with service date of 10/13/2023. First Aid Kit was observed to be complete. There are first aid supplies in Director's office. Fire Drill log was observed to be complete and current.

The facility has sufficient mats for napping. Each mat is occupied by only one child at time. CPR, Pediatric First Aid and Mandated Reporter Training certificates were observed to be current. The name of the child care center director or fully qualified teacher designated to act in the director’s absence has been reported to the Department. LPA reviewed a sample of children files and observed one file missing a written medical assessment of the child. LPA reviewed a sample of staff files and observed one file missing the health-screening report. The children's roster was available and current.

Director was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

Director was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for
drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP).
LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP.

LPA discussed the safe sleep regulations with Director and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed Director of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department.
