On 02/28/2020 at 12:40 PM, Licensing Program Analysts (LPA) Adrian Risher conducted an announced Case Management visit for the purpose of an inspection for a proposed capacity increase. LPA met with Charlotte Harper, Licensee and toured the facility inside and out. The facility is applying for a capacity increase from 08 to 14. Upon arrival, 1 infant and 2 children were observed napping in the bedrooms. Hours of operation are Monday through Friday from 6:00 AM – 12:00 AM. The fire clearance was granted on 09/16/19.

The single family home has a living room, dining room, kitchen, den, 3 bedrooms and 2 bathrooms. The daycare will consist of the den and 2 bedrooms to the right in the hallway. These areas were inspected by LPA. The other bedrooms and rooms are considered off-limits and inaccessible. Applicant is renting the home. Applicant provided property owner/landlord consent form with the application.

Licensee stated that the den is used for eating, watching tv, and teaching. The children nap in the 2 bedrooms to the rear of the home. LPA observed playpens and cots for napping. LPA observed high chairs and age-appropriate tables with chairs in the den. LPA inspected the bathroom and did not observe any medications or poisons that could pose a potential risk to children in care. The kitchen was inspected during the visit. LPA did not observe any knives or sharp objects, detergents or cleaning supplies that would pose a potential risk to children in care. These items were made inaccessible to the children. LPA observed a baby safety gate placed at both of the entrances of the kitchen. Licensee has put safety locks on the lower cabinets in the bathroom. LPA observed the kitchen cabinets without knobs. Licensee stated that the children do not have access to the kitchen. Licensee stated that the children use the bathroom to the rear of the home.

LPA observed age appropriate toys, furniture and activities for children while in care at the facility. Licensee stated that she moves the play houses to the area that the children are playing in. LPA observed 2 playhouses/kitchens in the hallway near the restroom. Licensee stated that the outside play area in the backyard is currently off-limits. LPA observed a broken slide in the backyard. Licensee stated that she is going to clean up the backyard before making it accessible to children in care. There are no bodies of water.

LPA observed a charged fire extinguisher (2-A:10-B:C). LPA tested the smoke detector and carbon monoxide detector. LPA observed detergents, cleaning supplies and medications inaccessible to children. Applicant has a first aid kit which includes band aids and a thermometer. LPA observed a wall heater with a gate around it to make it inaccessible to children in care. Licensee stated that she uses fans and ceiling fans to keep the home cool in warmer weather.

Applicant has current CPR, first aid, health and safety which expires 02/2022. Per the applicant, there are no firearms on the premises. Applicant reports that there are no pets in the home. LPA did not observe any baby walkers, exersaucers or bouncers.



The parent board will be posted for parents to view required information such as the license, parent's rights poster, personal rights, and emergency disaster plan. A copy of the children's roster will be kept accessible.

Copies of immunization records and mandated reporter training has been reviewed and completed. These documents were provided with the application.

A license to operate a Large Family child care home will be reviewed following final administrative review. No license will be issued today 02/28/2020.

The following was discussed with the licensee:

The licensee was informed to have a qualified assistant when the capacity exceeds 8 children. The assistant must be at least 14 years of age, but can not be left alone with the children in care. If the assistant is 18 years of age and older, the assistant must have current Adult/Infant & Pediatric First Aid certificates if left alone with children while the licensee is out of the home. Each assistant must also have TB clearance, valid criminal record clearance, immunizations and be associated to the facility license.

Applicant was informed that all infants must be placed on their backs when sleeping to prevent S.I.D.S. (Sudden Infant Death Syndrome). The applicant was provided a handout on Safe Sleep. No smoking is allowed on a day care premises. Never shake a baby to prevent Shaken Baby Syndrome. Only children eating may be in high chairs. Applicant was notified that it is required to wash hands after every diaper change.
