On 06/01/2022 Licensing Program Analyst (LPA) Jillinda Chandler made an unannounced visit to the Rainbow Children's Academy, Corp. for the purpose of amending the allegation recorded on the licensing report dated 5/27/2022 and observe the facility for plans of correction issued on 5/31/2022.

Upon Arrival LPA Chandler met with Ebony Smith (office assistant) who provided a tour of the facility. LPA observed 55 preschool children under the supervision of 6 qualified teachers, and one aide. Later during the visit director Kelly Coleman joined LPA

LPA did not observe a posted copy of the licensing report dated 5/31/2022, LPA made a review of children files and did not observe an Acknowledgment of Licensing Reports (LIC 9224) in the reviewed file.

The licensee was advised that the Notice of Site Visit must be posted at the entrance of the facility for a period of 30 days. If facility was cited type A violations or complaint is found to be substantiated or unsubstantiated, a copy of the licensing report (LIC809 or LIC9099) must also be posted for 30 days. If these requirements are not met, civil penalties in the amount of $100 per violation will be assessed. Licensee must inform the parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility during the next 12 months via form LIC-9224 Acknowledgement of Receipt of Licensing Reports


The facility was sited and civil penalties issued. An exit interview was conducted and a copy of this report was given to director Kelly Coleman
