On 06/26/2024 at 8:50 AM an unannounced Annual/Random Inspection was conducted by Licensing Program Analysts (LPAs) Katrina Chicote and Peter Bishop. LPAs announced purpose of visit and met with a variety of Staff. Facility is currently licensed for a capacity of 92 and per Facility Roster currently have 73 enrolled. The program currently operates Monday thru Friday from 7:00 AM to 5:30 PM. LPAs singularly toured the facility both indoors and outdoors. Required postings were observed. LIC 125 was provided.

The facility is made up of four classrooms: RM-1, RM-2, RM-3A and RM-3B. At the initial start of the site visit, LPAs observed all children playing outside. LPAs observed RM-2 with 19 children and three staff playing in Outdoor Play Area 1 and RMs 1, 3A, and 3B playing together in another Outdoor Play Area 2 with 28 children and five staff. Total census observed was 47 children with eight staff at time of inspection.

Beginning at 9:10 AM, furniture and equipment were inspected for age appropriateness and good repair in each classroom. LPAs observed surface of classrooms to be safe and clean, and child size tables and chairs available for children with rounded edges and well maintained. LPAs observed learning stations for dramatic play, math, and science with a variety of age appropriate toys maintained free of loose and sharp parts and a library area with soft seating available for quiet play. Telephone service, heating, lighting and ventilation were evaluated. Cots were inspected and observations discussed. Per FR linens are provided by the facility and washed by weekly laundry service. Availability of drinking water was reviewed. LPAs observed drinking fountains available for children in some classrooms as well as children's personal water bottles available. There were cubbies observed as storage for children's belongings in hallways by bathrooms. Age appropriate sinks and toilets were inspected for availability, good repair, water temperature, toilet paper, area safety and sanitation. LPAs observed three bathrooms available totaling six toilets (2 urinals) and six sinks. Hazardous items including poisonous cleaning compounds were stored inaccessible to children. Per FR, only LAUSD approved disinfectants are present at the facility, LPA observed these items stored in teacher cabinets with lock available at time of inspection.

LPAs observed first aid kit is kept on mobile carts that can be taken outside, per FR each classrooms has their own cart and a larger first aid kit is available in a closet by RM-1. LPAs observed First Aid Kit supples in RM-1 closet. Per FR, they currently have one child (C1) that require medicine. LPAs observed medicines stored in RM-1 closet which is centrally located but inaccessible to children as it requires a key to open. LPAs observed medicines for children in original packaging with unaltered label and conducted record review to ensure completion of required documents. At 10:40 AM, LPAs observed carbon monoxide detector present in the Kitchen and was observed operable by green indicator light. LPAs observed smoke detectors and fire extinguishers are present in the facility at time of inspection. LPAs discussed requirement to have fire extinguishers serviced annually and to conduct regular checks of smoke and carbon monoxide detectors to ensure they are in operable condition. Disaster drill log was observed posted and last drill conducted was on 06/24/2024. Isolation area for sick children is in School's Office and will have full supervision until child is picked up by Authorized Representative.

Snack/lunch menus are provided to parents digitally and QR code was observed posted. Facility provides Breakfast, Lunch, and PM Snack. Food is delivered by LAUSD Central Kitchen and distributed at the facility. Food preparation areas were toured for safety, cleanliness and proper equipment. LPAs observed trash cans with tight fitting lids available at time of inspection.

Beginning at 9:40 AM, outdoor equipment was inspected for safety, cushioning material, good repair and age-appropriateness. LPAs observed two outdoor play areas available at time of inspection. LPAs observed surface of outdoor play areas to be safe and maintained for activities planned such as concrete bicycle path and cushioning material below multi-level play structure. LPAs observed multi-level play structure to have climbing equipment and slide that is age appropriate and maintained. LPAs observed a variety of age appropriate toys available for dramatic play, manipulatives, and art that have all been well maintained and made for outdoor use. Required shade and fencing were inspected. LPAs observed a variety of shaded areas available for children with seating available at time of inspection. LPAs advised that the children need to be within the direct care and supervision, including visual supervision of the teacher(s) at all times. The children have access to drinking fountains and their personal water bottles during outside play, which LPAs observed available through out play yard. Play area was inspected for hazards and inaccessibility to bodies of water; no bodies of water or hazards observed.

Teacher-child ratios were observed and staff names recorded. Care and supervision was evaluated to determine if the basic needs of children are met and appropriate. Sign-in and out sheets and procedures were reviewed and discussed. Personal Rights of children were observed by LPA.

Beginning at 10:50 AM, staff and children’s records were reviewed for completeness. Inspection of required forms was made and documented on the LIC 857 for children and the LIC 859 for staff. LPAs did not review confidential staff files during the inspection due to the staff confidential files being stored at the administrative office. CPR card expires on 06/2026 for Staff #1. 6 of 6 staff files reviewed did have proof of current AB1207 Mandated Reporter Training certificate on file. All staff and employees have obtained a criminal record clearance or criminal record exemption as a condition of employment with the Los Angeles Unified School District (LAUSD).

The following were discussed:
LPA advised of the requirement to report Unusual Incidents and/or injuries to the parent/guardian and to CCL within the time frame specified by the regulation, this includes any changes as soon as they occur such as construction, remodeling, telephone number changes and/or if you move from your location.

LPA advised the licensee to access forms, regulations and quarterly updates on the Child Care Licensing website at: www.ccld.ca.gov.

To improve the quality and value of the new inspection process, a survey may be sent to the email address provided. Please complete the survey and share your inspection experience. Facility Representative was reminded that all adults 18 and over in the facility, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed facility. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

CCC COMPLETED TESTING AND NO LEAD EXCEEDANCES:
Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test.

LPA referred Facility Representative to the Department website for lead:

Incidental Medical Services (IMS) - Child Care Centers
Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. MyChildCarePlan.org – Centers and Family Child Care Homes Licensee [or facility representative] was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.


The facility was found in compliance per Title 22 regulations, there will be no deficiencies cited today only advisories given, 06/26/2024.

A notice of site visit was given and must remain posted for 30 days.
