2/16/2023 Licensing Program Analysts (LPA) Judy Laureano conducted unannounced Annual Required Inspection at 807 Arizona Avenue, Santa Monica 90401 LPA was greeted by Nereida Garcia, Assistant Director, who guided LPA Laureano on a tour of the facility inside and outside.

LPA observed the following:
Johnny 1 Classroom- 15 children and 3 staff members
Johnny 2 Classroom- 10 children and 2 staff members
Johnny 3 Classroom- 13 children and 3 staff members
Ruby 1 Classroom- 11 children and 2 staff members
Ruby 2 Classroom- 19 children and 3 staff members
Ruby 3 Classroom- 10 children and 4 staff members

During today’s inspection, Poppies Classroom had no preschool children present. Facility does use the classroom for a parenting group. Children and parents from this classroom do not comingle with the preschool classroom.

Facility currently operates Monday through Friday 8:00 a.m. to 4:00 p.m. During today’s inspection, 78 children were present. The preschool serves children ages 2 years to entry into Kindergarten. Children will utilize classrooms located on the 1st and 2nd levels only.

Currently Site Director is Jennifer Shpall and a director’s packet will be submitted by 2/28/2023.

Facility has a working telephone service and LPA confirmed the telephone number is 310-451-4600.

LPA toured and inspected the facility in accordance with the facility sketch. Teacher/child ratios are maintained, and adequate supervision is being provided during this visit:

There is no swimming pool or other bodies of water on the premises. There are no firearms or ammunition allowed or stored on the premises.

Disinfectants and cleaning solutions are kept in the children’s bathroom- top shelf- made inaccessible to the children in care. Assistant Director confirmed facility has a designated cleaning person that comes and cleans the facility on weekends. Teachers are responsible for the daily cleaning in between classroom activities. Facility contracts monthly with outside pest control vendor to ensure facility stays pest free. Facility is free of flies, insects and rodents. No poisons were observed during the inspection

First Aid Kit and disaster kits was observed in each classroom. Medication is stored in classrooms where children are enrolled and need medication such as an Epi pen. All necessary forms are stored with medication in the top shelf inaccessible to the children in care.

Furniture and equipment are in good condition, free of sharp, loose or pointed parts. Playground equipment is in safe condition, free of sharp, loose or pointed parts.

The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. LPA observed shaded area in the outdoor space with age appropriate play equipment and toys. Areas around high climbing equipment have cushioning material to absorb falls. Teachers ensure playground and outdoor equipment is safe and free of hazard before children use the area.

All toilets and handwashing facilities are in safe and sanitary operating condition. Floors in the facility are clean and safe. Solid waste storage containers have tight-fitting covers and are in good repair.

All kitchen, food preparation and storage areas are clean, free of litter/rubbish and free of rodents/vermin. Facility does not provide meals or snacks. Families are responsible for providing meals and snacks in appropriate containers such as lunch boxes with ice packs and/or thermos. All food is protected against contamination and any contaminated food is discarded immediately. Drinking water is available both indoors and outdoors, filter water is provided in each classroom and children are encouraged to bring their refillable water bottle. Children bring their refillable water bottle and teachers are responsible for refilling.

There is a working fire extinguisher, smoke detector, carbon monoxide detector and adequate heating and ventilation for safety and comfort. LPA observed a centralize fire and smoke detector alarm with fire sprinkler.

Prior to working or volunteering in a licensed child care facility, all individuals subject to a criminal record review have received a criminal record clearance or exemption. Upon notification from the Department, the licensee will comply and act immediately to terminate the employment of, remove from the facility or bar from entering the facility for any person it is deemed necessary while the Department considers granting or denying an exemption. Capacity and limitations as specified on the license are being maintained.

LPA reviewed 6 staff files and observed one file to be incomplete and a technical advisory was issues. K. Echeverria needs current Mandated Reporter Training . LPA provided facility admin with a copy of LIC311A and LIC 125 to use as a guide to audit files and program. Assistant Director's Pediatric CPR and First is was completed on 8/27/2021 and Mandated Reporter was completed 5/5/2022.

LPAs reviewed children’s files and observed files were complete with contact information for authorized representative and or relatives or others who can assume responsibility for the child and medical assessment.

The person who signs the child in/out of the facility shall use their full legal signature and record the time of day. Facility currently uses an electronic sign in/out sheet- Brightwheel- and a print out is available for LPA’s review.

All children are under supervision, including visual supervision, of a teacher at all times. Facility maintains a ratio of one teacher supervising no more than 12 children in care.

Facility representative, Jennifer Shpall and Nereida Garcia, was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Incidental Medical Services (IMS) currently being provided. Licensee is aware that an IMS plan is required to be submitted to the licensing office if they provide any of these services. Information regarding Americans with Disability Act (ADA) can be obtained by contacting US Department of Justice toll free ADA Information line at (800) 514-0301(voice), (800) 514-0383 (TDD) and website link https://www.ada.gov/childqanda.htm.



LPA and facility representative discussed the Community Care Licensing website www.ccld.ca.gov which will provide access to Provider Information Notices (PINs), Quarterly Updates, COVID-19 Information and Resources, Mandated Reporter Training, Safe Sleep in Child Care, Lead Poisoning Facts, Forms and Regulations.

A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report with Notice of Site Visit was reviewed with the licensee facility representative, Jennifer Shapall and Nereida Garcia.
