New Immunization Requirement: Law enacted by SB 277, beginning January 1, 2016, personal beliefs exemptions will no longer be an option for the vaccines that are currently required for entry into child care or school in California. Personal beliefs exemptions already on file will remain valid until the child reaches the next immunization checkpoint.
New Appeal Process: A licensee may file an appeal, in writing 15 business days from the date of receiving the penalty assessment. Appeal may be submitted and mailed to the following address:
300 North Continental Blvd. suite # 290 A
El Segundo, CA 90245
The licensee was advised that the Notice of Site Visit must be posted at the entrance of the facility for a period of 30 days. If a serious violation is cited, a copy of the licensing report (LIC809 or LIC9099) must also be posted for 30 days. If these requirements are not met, civil penalties in the amount of $100 per violation will be assessed. Copies of the reports must also be provided to each parent when a serious deficiency, Type A, is cited (LIC9224).
The facility was found to be NOT in compliance per Title 22 regulations,
The above facility shall be cited a Type B deficiency, today, 12 /18/19.
*101229.1(a)(1)(b) Sign In and Sign Out
(a)In addition to the sign-in procedure requirement of Section 101226.1(b), the licensee shall develop, maintain, and
implement a written procedure to sign the child in/out of the child care center that shall, at a minimum, include the following:
(1) The person who signs the child in/out shall use his/her full legal signature and shall record the time of day.
(b) The person who brings the child to, and removes the child from, the center shall sign the child in/out.
LPA observed missing signatures in Sign In/Out record sheets. The above requirement is not met as evidenced by observation and record reviews conducted during today’s inspection. The director failed to ensure that the above records have appropriate and complete signature. This poses a potential risk to the health and safety of the children in care. Director agrees to send proof of correction, missing signatures obtained and email reminder to parents by 12/18/2019, end of business day. A copy of this report along with a Notice of Site Visit were issued and explained to the licensee.
An exit interview was discussed and conducted with preschool director, Maduka Wijayaratna , including, but not limited to: Provider Rights, Appeal Procedures and Agency's Consultative Role.
