On 12/02/2021 at 12:35 PM, Licensing Program Analyst (LPA) Antonio Almanza, conducted an unannounced Annual Required Inspection for the preschool license. LPA met with Secretary. Floating Site Supervisor Lizbeth Renova arrived at the facility at approximately 1:45 PM, and provided LPA assistance. Staff 1 guided LPA on a tour of the facility indoors and outdoors. Days and hours of operation are Monday to Friday 7:30 AM to 5:30 PM. LPA verified facility phone number 323-731-1981.

The facility is on the premises of Hope Los Angeles Church located on the corner of Somerset Dr and Jefferson Blvd. The facility is entered on the double doors located at the back of the church on Somerset Dr. The facility is composed of one large room, bathroom (two toilets and two sinks), office and kitchen. The facility Playground has an on entrance on Jefferson Blvd on the right hand side of the church.

During this time LPA us unable to complete Inspection due to time constraints. LPA will return to complete Inspection.

A copy of this Report, Notice of Site Visit, and Appeal Rights were explained and provided to Lizbeth Renova, Floating Site Supervisor.
