An unannounced Required – 1 Year Inspection was conducted by Licensing Program Analyst (LPA) Katrina Chicote to the above facility. LPA announced purpose of visit and initially met with Brittie Crawford, Licensee, who provided a guided tour of the facility both indoors and outdoors. Director, Debora Cornejo, later joined inspection. Facility is currently licensed for a capacity of 35, per Director there are 31 preschoolers enrolled. The program currently operates Monday thru Friday from 7:30 AM to 5:30 PM and is made up of three classrooms: Kahlo Room (3-4 year olds), Rivera Room (3-4 year olds), and Dali Room (2.5-3 year olds).

At the initial start of the site visit, there is a total census of 25 children and a total census of six staff present. Upon arrival to facility, LPA observed children playing in outside play yard.

Beginning at 11:35 AM, outdoor equipment was inspected for safety, cushioning material, good repair and age-appropriateness. Required shade and fencing were inspected. LPA observed two separate play areas for 3-4 year olds and 2.5-3 years old separated by fencing. LPA observed 3-4 year old play area to have multi-level plastic play structure with cushioning material below to absorb fall with surface maintained and plastic dramatic play equipment available for children to be well maintained at time of inspection. LPA observed a garden in rear of play area to be fenced off. LPA observed 2s play area to have surface that is well maintained with age appropriate toys available. LPA advised that the children need to be within the direct care and supervision, including visual supervision of the teacher(s) at all times. LPA observed the children have access to their personal water bottles for drinking water outside. Play area was inspected for hazards and inaccessibility to bodies of water; no bodies of water or hazards observed at time of inspection.

Beginning at 11:50 AM, furniture and equipment were inspected for age appropriateness and good repair. LPA observed surface of classrooms to be safe and clean, and age appropriate tables and chairs available for children well maintained. LPA observed learning stations for dramatic play, math, and science with toys well maintained free of loose, sharp, and pointed parts and a library area with soft seating available for quiet play.

Telephone service, heating, lighting and ventilation were evaluated. There were cubbies observed as storage for children's belongings and hooks for children's backpacks. LPA observed cots laid out at time of inspection that does not hinder walkways or exits/entrances. Per Director, linens are provided by the families and taken home weekly for washing. Availability of drinking water was reviewed. LPA observed water coolers available for children and children's personal water bottles. Age appropriate sinks and toilets were inspected for availability, good repair, water temperature, toilet paper, area safety and sanitation. LPA observed bathroom available for Dali Room that had two toilets and two sinks and a shared bathroom for Kahlo and Rivera that had three toilets and three sinks.

A first aid kit is kept in lobby. LPA tested carbon monoxide detectors and observed to be in operable condition. LPA observed smoke detectors and fire extinguishers are present in the facility. Disaster drill log was available and last drill conducted on 09/21/2023. Per Director, isolation area for sick children is in upstairs multi-purpose room. LPA observed mats available for sick child and nearby bathroom available. LPA observed medicine stored for C1 in Dali Room that is stored according to Title 22 Regulation requirements.

Per Director, facility provides AM and PM snack and families bring in their own lunches from home. LPA observed lunch boxes available for children. Snack menus were reviewed and observed posted and per Director menus are also provided digitally to parents via app ClassDojo. Food and snacks were reviewed for availability, quantity and appropriateness to children in care. Food preparation areas were toured for safety, cleanliness and proper equipment.

Teacher-child ratios were observed and staff names recorded. Care and supervision was evaluated to determine if the basic needs of children are met and appropriate. Sign-in and out sheets and procedures were reviewed. Per Director, they utilize a digital sign in via QR code by entrance. LPA observed only 12 of 25 preschoolers present were signed in during today's inspection. Requirements discussed. Personal Rights of children were observed by LPA.


Beginning at 12:30 PM, staff and children’s records were reviewed for completeness. Inspection of required forms was made and documented on the LIC 857 for children and the LIC 859 for staff. Criminal Record Clearances were reviewed for adults. CPR card expires on 03/2025 for Staff #1. Inspection of required forms made. 2 out of 5 staff file reviews conducted did not have proof of the AB 1207 Mandated Reporter Training certificate on file at time of inspection.

Incidental Medical Services:
This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. CCC COMPLETED TESTING AND NO LEAD EXCEEDANCES:
Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP).
LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP.

Licensee (or facility representative) was reminded that all adults 18 and over in the facility, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed facility. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPA advised changes should be reported to the Department as soon as they occur such as construction, remodeling, telephone number changes and/or if you move from your location.

LPA advised the licensee to access forms, regulations and quarterly updates on the Child Care Licensing website at: www.ccld.ca.gov.

LPA advised of the requirement to report Unusual Incidents and/or injuries to the parent/guardian and to CCL within the time frame specified by the regulation.

Based on this information, the following deficiency on the attached LIC 809D are being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children’s health & safety.

A notice of site visit was given and must remain posted for 30 days.
